Job summary
We are looking for a compassionate and enthusiastic Acute Illness Nurse to join our friendly and supportive clinical team.
Our Acute Illness Nurse will play a critical role in the management and treatment of patients presenting with minor illnesses and acute conditions within a general practice setting. This role involves the assessment, diagnosis, and treatment of a wide range of acute illnesses, ensuring timely and effective care for patients. The Acute Illness Nurse works independently but is supported by GPs, nurse practitioners, and other healthcare professionals to provide high-quality, patient-centred care.

The post-holder will be responsible for assessing, diagnosing and treating patients with acute conditions. You will also provide expert professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence.
We operate a full care navigation appointments system, via a range of communication channels including telephone consultations, e-consultation, and face-to-face appointments.
We have 2 local branch sites in Kimberworth and Ridgeway, you may be required to work at these sites as part of a rotation.
Main duties of the job
You will have the ability to make professional autonomous decisions when consulting with patients with undiagnosed acute illness, and make assessment of their health care needs. As an acute illness nurse you will be seeing patients independently in Face-to-Face or telephone appointments. Within patient consultations you will use highly-developed nursing knowledge and skills such as physical examination, applied pharmacology and evidence-based prescribing. Maintain accurate and timely patient records using clinical IT systems (e.g. SystmOne).
Work collaboratively within the practice nursing team, GPs, and other healthcare professionals

Job description Job responsibilities
The postholder will provide high quality clinical care across face to face, telephone, and where appropriate, home visit consultations. They will assess, diagnose, and manage a broad range of acute presentations and long-term conditions, offering evidence-based treatment plans, health promotion advice, and ongoing support for patients with chronic and acute illnesses.
The role includes requesting and interpreting relevant investigations, contributing to same day urgent care and triage services, and ensuring all clinical encounters are documented accurately and contemporaneously using SystmOne.
Working as part of a supportive multidisciplinary team, the clinician will participate in clinical meetings, audits, and service improvement initiatives, while maintaining compliance with NHS, CQC, and professional standards.
Strong clinical assessment and decision-making abilities are essential, alongside excellent communication and interpersonal skills. The ability to work both independently and collaboratively is key, as is the flexibility to travel between sites when required. A compassionate, patient centred approach is fundamental, with the capacity to adapt communication style to meet diverse patient needs and to produce clear, concise, and clinically robust documentation, referrals, and safety netting advice.
While not essential, previous experience within a GP surgery, a prescribing qualification, and training in minor illness or minor injury would be advantageous. Familiarity with SystmOne and a good understanding of local NHS pathways and community services would further strengthen the candidates ability to deliver safe, effective care within the practice.

Qualifications Essential
Degree or equivalent in Nursing and on NMC register. Experience of independently dealing with day to day acute illness in primary care, community or in Accident and emergency.
Desirable
Ability to identify determinants of health in the local area. Knowledge of public health issues in the local area. Awareness of local and national health policy
