Salary: Up to £56,000.00 per year (depending on level of experience, training and qualification)

We have an opportunity for a Senior Clinical Pharmacist to join their team of staff in the Pharmacy department.

This is a full time role for 37.5 hours a week. The role holder will be required to cover a flexible shift pattern.

Main duties of the job

Duties of this role include:

Provide expert clinical pharmacy input as part of the multidisciplinary team, conducting medicines reconciliation, clinical screening, and therapeutic drug monitoring.

Optimise medicines use through evidence-based interventions, ensuring the safe and effective management of long-term and acute conditions.

Support prescribing decisions through clinical assessment, risk evaluation, and formulary guidance.

Lead on implementation of medicines optimisation strategies, ensuring prescribing is cost-effective, safe, and aligned with national/local guidelines.

Contribute to the development and review of clinical guidelines, protocols, and policies.

Monitor and report on medicines usage, compliance, and safety incidents, participating in root cause analysis and quality improvement.

Applicants should meet the following criteria:

GPhC-registered Pharmacist.

Significant post-registration experience in clinical pharmacy, preferably in a hospital setting.

Strong clinical knowledge and experience in medicines optimisation.

Excellent communication, leadership, and multidisciplinary collaboration skills.

Experience in mentoring or supervising junior staff.

Must have BOPA Passport and Clinical Pharmacy Diploma.

Advanced Clinical Practice

Provide expert clinical pharmacy input as part of the multidisciplinary team, conducting medicines reconciliation, clinical screening, and therapeutic drug monitoring.

Medicines Optimisation and Governance

Lead on implementation of medicines optimisation strategies, ensuring prescribing is cost-effective, safe, and aligned with national/local guidelines.

Contribute to the development and review of clinical guidelines, protocols, and policies.

Monitor and report on medicines usage, compliance, and safety incidents, participating in root cause analysis and quality improvement.

Education, Mentorship, and Support

Provide supervision, mentorship, and support to junior pharmacists, pre-registration trainees, and pharmacy technicians.

Contribute to training programmes for pharmacy staff and support wider clinical education for nurses and prescribers.

Lead clinical audits, case reviews, and service evaluations within your specialty.

Collaborative Working and Clinical Leadership

Act as the lead pharmacist for designated specialties or clinical areas, working closely with consultants, nurses, and AHPs.

Attend ward rounds, clinical meetings, and governance forums, offering proactive input into patient care plans.

Represent pharmacy in multidisciplinary quality initiatives and clinical service reviews.

Policy, Safety, and Compliance

Ensure compliance with all regulatory and legal frameworks including GPhC, CQC, MHRA, and controlled drugs legislation.

Support implementation of electronic prescribing systems (where applicable) and contribute to digital medicines optimisation.

Actively participate in the management and review of medication incidents and safety alerts.

Service Development and Innovation

Lead or support the development of new pharmacy services, clinical pathways, and patient-centred care initiatives.

Identify areas for service improvement, efficiency, and innovation, contributing to business cases and strategic planning.

Help shape a culture of clinical excellence, safety, and continuous improvement.

WHAT DO YOU HAVE?

Essential:

GPhC-registered Pharmacist.

Significant post-registration experience in clinical pharmacy, preferably in a hospital setting.

Strong clinical knowledge and experience in medicines optimisation.

Excellent communication, leadership, and multidisciplinary collaboration skills.

Experience in mentoring or supervising junior staff.

Desirable:

Postgraduate qualification in Clinical Pharmacy (e.g. Diploma or MSc).

Independent Prescriber qualification.

Experience in specialist areas (e.g. oncology, surgery, critical care, cardiology).

Experience in audit, policy development, and quality improvement projects.

Familiarity with EPMA systems and digital health tools.
