Job summary

This is an exciting new role suitable for a qualified Health Care Assistant who will provide planned and unplanned care for practice patients in their own homes.

Phlebotomy and Long Term Condition Reviews.

The role holder will be an experienced Health Care Assistant currently working in Primary or Community Care. They will be expected to work autonomously within the nursing team to deliver excellent patient care, in the community.

Main duties of the job

Duties and responsibilities

Work in partnership with individuals, families, and carers to deliver patient-centred care To monitor care in line with practice protocols Undertake face to face consultations at the patients home to deliver care. Make referrals to other services or professionals for investigations or treatments as appropriate. Undertake wellbeing checks for long term condition reviews Administer adult immunisations and vaccinations in line with the national and local policies. Provide treatment room duties, including wound care, NHS Health Checks, ear irrigation. Provide training and support for nursing students, trainee GPs and other health care professionals within the practice and Primary Care Network. To undertake administration tasks such as stock, ordering, room fill as allocated.

The post holder will be expected:

To work autonomously, to provide holistic care to patients. Plan visits ensuring they are undertaking in a timely manner in line with practice protocols. Review patients following practice protocols, identifying areas for support offering advice, guidance or escalation as needed. To seek out opportunities for practice development within their own scope of practice. To support the learning and development of colleagues, students, professionals and patients within the surgery and the Primary Care Network. To embed new/changing policies, protocols, and procedures within the nursing team to meet the needs of the practice. To support the nursing team to meet local and national targets relating to patient care. To hold a full, clean driving license and appropriate car insurance. Responsibilities within Practice

The practice is committed to supporting and promoting opportunities for staff to maintain their health, well-being and safety. You have a duty to take reasonable care of health and safety at work for you, your team and others, and to cooperate with employers to ensure compliance of health and safety requirements. All personnel are to comply with the Health and Safety at Work Act 1974, Environmental Protection Act 1990, Environmental Act 1995, Fire Precautions (workplace) Regulations 1999 and other statutory legislation. Furthermore, it is expected to be vaccinated in line with current guidance.

Staff are required to dress appropriately in the workplace, in uniform if applicable. Consideration to infection prevention and control should be made for clinical roles.

The practice has a no-smoking policy. Smoking is not permitted on site and staff should not smoke in identifiable uniforms or with name badges visible. Support can be offered to members of staff who wish to stop.

Car parking is available on site and free of charge.

An Enhanced Disclosure from the Criminal Records Bureau will be requested in the event of a successful application in line with the Rehabilitation of Offenders Act 1974.

A supportive 6 month probationary period will apply.

Person Specification

Qualifications Essential

GCSE grade A-C in English and Maths or equivalent Ideally you will have completed the HCA Care Certificate or NVQ Level 2 or 3 in Health and Social Care (or equivalent qualification/experience). Qualified phlebotomist with recent experience

Experience of working with the general public Experience of working in a healthcare setting Ability to record on clinical notes Excellent communications skills and including excellent IT skills Effective time management Ability to work as a team and autonomously Good interpersonal skills Ability to follow clinical policy Polite and confident with a flexible approach. An individual who is self motivated and has excellent problem solving skills. Operates with high integrity and loyalty Commitment to ongoing professional development Punctual and committed member of the team Ability to work under pressure You will be expected to undertake a DBS check.

Experience of working in Primary Care Up to date training in LTC reviews and NHS Healthchecks
