Job summary
Due to a retirement, we are looking for an enthusiastic and confident Frailty Nurse to join our friendly Collaborative Care Team (CCT) .
The successful candidate will be passionate about supporting patients living with frailty and committed to providing outstanding care in patients own homes, ensuring their wellbeing in the later stages of life (with dedicated GP support from our Frailty Lead).
You must be a qualified nurse with excellent communication skills, as you will be liaising with a wide range of clinical and non-clinical teams, diverse patient groups, and their families.
The role is approximately 15 hours per week; however, this can be negotiated. Salary is also negotiable and dependent on experience and skills.
Main duties of the job
You will assist medical personnel in the care of frail patients, including treatment, preventative care, screening, and patient education. The role will involve visiting housebound patients and care homes, therefore the use of your own transport will be essential. Please see the job description for the full details.

JOB TITLE: Frailty Nurse
MAIN PURPOSE OF POST:To assist in the care of vulnerable patients that may be housebound or in a Care Home. The role will involve assessments, maintenance of care plans, health management and vaccinations. The role will require visiting patients using own vehicle.
RESPONSIBLE TO:Nurse Manager and Assistant Managers
MAJOR DUTIES AND RESPONSIBILITIES:
1. To monitor long term conditions either at home or in a care home for patients on the CCT list
2.To support the partnership in strategic planning, audit and research relating to primary care services within the practice.
3. Liaison with other professionals.
4. Carry out injections as required.
5. Carry out home visits.
6. Any other delegated duty considered appropriate for the post
7. Professional development
1) To complete and maintain care plans for patients on the CCT list
i) Complete care plans for all new care home residents
ii) Complete care plans for all new patients added to CCT list.
iii) To update care plans on an annual basis (including creating and reviewing ReSPECT forms)
2) To provide Vaccinations
i) To assist in flu/COVID vaccination programme both at Prospect House and at patients home
ii) To assist in any other vaccination programme as required
3) Liaison
i) To attend practice care team meetings
ii) Work with practice and care coordinators in respect of frail patients
4) To provide patient care
i) Request blood tests and take bloods.
ii) Advise patients on general health care and minor ailments with referral to general practitioners as necessary.
iii) Carry out physical examinations and observations when required
iv) Liaise with practice safeguarding team when required
v) Refer patients to appropriate clinical or social departments/agencies
vi) Discuss social support
5) To participate in the training of all new members of the practice Registrars and students on placement within the practice.
Participate in the education and training of students of all disciplines and the induction of all members of practice staff with special responsibility for nursing staff.
6) Place of work
i) Based at Eskdaill, there will be a requirement to regularly visit patients in either their own homes or care homes
ii) There is a requirement to use personal transport to visit patients, and allowance will be paid based on mileage used. There will be a requirement for the vehicle to have business use on the insurance
7) Professional development
i) Maintain continued education including attendance at courses and study days as deemed useful or necessary for professional development.
ii) Maintain a written professional development plan and maintain an up-to-date portfolio which meets the requirements of registration with the NMC
iii) Attend annual personal appraisal
iv) Attend regular clinical supervision session
SPECIAL REQUIREMENTS OF THE POST:
