Main duties of the job
The Practice Nurse will provide high-quality nursing care to patients within the GP practice, including chronic disease management, health promotion, immunisations, cervical screening, wound care, and routine nursing procedures. The role involves supporting patients with long-term conditions such as diabetes, asthma, COPD, and hypertension, while promoting healthy lifestyles and preventative care.
Duties also include maintaining accurate clinical records, participating in quality improvement activities, and ensuring compliance with infection control and safeguarding procedures.

Job SummaryThe Practice Nurse will provide high-quality nursing care to patients within the GP practice, supporting the delivery of safe, effective, and patient-centred services. The role includes chronic disease management, health promotion, immunisations, cervical screening, wound care, and general treatment room duties.
The successful candidate will work collaboratively with the multidisciplinary team to improve patient outcomes and support the ongoing development of practice services.
Main Duties and ResponsibilitiesClinical Responsibilities
Deliver routine nursing care and treatment room services to patients.
Administer childhood and adult immunisations and vaccinations.
Undertake cervical cytology screening in line with national guidance.
Carry out wound care, dressings, ear irrigation, and suture removal.
Perform health assessments and NHS health checks.
Support the management of long-term conditions including:
Diabetes
Asthma
COPD
Hypertension
Undertake chronic disease monitoring and patient reviews.
Provide travel health advice and vaccinations where appropriate.
Carry out procedures such as:
Blood pressure monitoring
ECGs
Spirometry
Phlebotomy
Administer prescribed medications and injections safely.
Maintain accurate and contemporaneous patient records using the clinical system.
Health Promotion
Promote healthy lifestyle choices and preventative healthcare.
Provide advice on smoking cessation, diet, exercise, alcohol reduction, and weight management.
Support patients in self-management of long-term conditions.
Participate in health campaigns and screening programmes.
Infection Control and Safeguarding
Adhere to infection prevention and control policies.
Ensure safe handling and disposal of sharps and clinical waste.
Maintain cleanliness and stock levels within clinical areas.
Recognise and report safeguarding concerns appropriately.
Work in accordance with practice policies, clinical governance, and NHS guidelines.
Administrative Responsibilities
Maintain accurate clinical documentation and coding.
Participate in audits, quality improvement initiatives, and QOF activities.

Person Specification Essential
Registered Nurse with current Nursing and Midwifery Council registration.
Experience in primary care, community nursing, or general practice nursing.
Knowledge of chronic disease management and immunisation programmes.
Excellent communication and organisational skills.
Ability to work independently and as part of a multidisciplinary team.
Desirable
Cervical cytology qualification.
Independent prescribing qualification.
Spirometry training.
Experience using EMIS or SystmOne clinical systems.

Qualifications Essential
Essential Qualifications Registered Nurse qualification (Adult Nursing). Current registration with the Nursing and Midwifery Council (NMC). Evidence of continuing professional development (CPD). Up-to-date mandatory training relevant to primary care nursing. Desirable Qualifications Cervical Cytology (Smear) qualification. Chronic Disease Management training (e.g. Diabetes, Asthma, COPD). Immunisation and Vaccination certification. Travel Health qualification. Spirometry training/certification. Independent Nurse Prescriber qualification (V300). Mentorship or teaching qualification. Basic Life Support (BLS) and Anaphylaxis training.
