What you will do in this role:

Deliver neuro-rehabilitation programmes in outpatient, home, and community settings under the guidance of qualified therapists. Support Physiotherapy and Occupational Therapy interventions. Monitor patient progress and contribute to therapy planning within agreed guidelines. What we offer:

Main duties of the job

To carry own patient caseload, assessing and treating patients as allocated bysenior therapist. Monitor patient and progress according to patient care plan, in-order to maximise each individual patient's functional ability. To carry out routine assessments as well as more skilled assessments (selectedneuro patients, simple stairs assessments, discharge from acute settings inconjunction with the Stroke Supported Discharge Team) To undertake specific skilled support work to aid the rehabilitation of patientsliving in their own homes. Where patients have complex needs, this is carried outin conjunction with qualified clinicians. Enable the delivery of effective and efficient rehabilitation teamwork to patientswhilst providing appropriate support and communication to relatives and carers. To participate in creating a culture of patient centred rehabilitation andenablement rather than being cared for and use every opportunity to promote ahealthy lifestyle and prevent accidents.

Person Specification

Qualifications Essential

GCSE or equivalent English and Maths (or equivalent) Proven broad based educational achievement Additional relevant training to diploma level or equivalent through short courses or experience

Desirable

Educated to NVQ 3 level or equivalent in a relevant subject i.e. Administration, IT or Health/ or Social Care or equivalent level of qualification or significant equivalent previous proven experience.

Experience/ Knowledge Essential

oDemonstrable experience in dealing with the public and dealing with sensitive and confidential information. Experience working with patients, clinical staff and carers Experience of working in a team

Desirable

oExperience of working within the NHS, social care or the voluntary sector. oKnowledge of health and social care or the voluntary sector.

Skills Essential

oClear communicator with excellent verbal, writing, data entry and presentation skills; capable of constructing and delivering complex and clear information/ instructions to staff and service users. oAble to work on own initiative, organising and prioritising own and others workloads to changing and often tight deadlines. Intermediate knowledge of IT systems, databases and software programmes such as Outlook, Word, Excel, PowerPoint.
