Rightcare Barnsley is a Single Point of Access, Transfer of Care Hub and Clinical Advisory Service based in Barnsley, we work collaboratively with Barnsley Hospital, SWYT Community Services, Barnsley ICB, Local Authority and Primary Care services.
We have a part time vacancy for a Registered Nurse to join the well-established team of Nurses and Call Operators to support our work within Barnsley.
Our role includes preventing avoidable admissions and supporting early discharge out of the hospital. The RightCare services are a key element of Urgent and Emergency Care delivery, we are a highly regarded, dynamic team with an established reputation.
RightCare is operational seven days a week, 12 hours a day between 8am 8pm.
This is a very exciting opportunity, for experienced Registered Nurses to join our continually expanding service and be involved in its ongoing development.
Main duties of the job
As a Registered Nurse your work will be split between the RightCare call centre based at the Kendray Hospital site and Barnsley Hospital. You will give clinical advice to referrers, assess and triage patients, make onwards referrals to the correct level of care. You will work closely with Neighbourhood Nursing services, Yorkshire Ambulance Service, Community Services, Adult Social Care, Care Homes and Barnsley Hospital services such as SDEC and the Emergency Department. Whilst in the call centre you will be responsible for contacting Virtual Ward patients via telephone, digital monitoring, assessing and participating in the virtual MDT meetings where patients are discussed with Community, Hospital and Social Care colleagues.
You will spend part of your working week at Barnsley Hospital assessing patients who have been referred to the Virtual ward and processing the admission. Community Nurses and other MDT members will provide direct patient care in the patients own home, the RightCare Virtual ward Nurses (this post) will remotely support them to do so.
