Job summary
This is an excellent opportunity to join Orion PCN as a Social Prescriber and play a key role in improving the health and wellbeing of our local Leicester City communities. We are looking for an enthusiastic and motivated individual to join our expanding team supporting patients across the PCN to take control of their health and access the right support at the right time.
As a Social Prescriber you will work holistically with patients to understand what matters to them promote positive lifestyle changes and help prevent ill health through behaviour change techniques. You will connect individuals to a wide range of community based services voluntary sector organisations and local activities that can support their physical emotional and social wellbeing. The role also contributes to reducing health inequalities by ensuring people receive personalised accessible and culturally sensitive support.
This position requires access to transport and the ability to travel between PCN practices visit patients in their homes and attend community events when needed.
Main duties of the job
The Social Prescriber will support patients to identify their needs, set personalised goals, and access appropriate community services, voluntary sector support, statutory services and healthrelated resources. The role empowers individuals to take greater control of their health and wellbeing while helping to reduce inappropriate clinical workload within General Practice.
The postholder will build strong relationships with local infrastructure organisations, community groups and support services to maintain an uptodate understanding of the local offer. They will promote social prescribing across the PCN and wider agencies, ensuring patients, families and carers can access personalised, communitybased support.
Key responsibilities include:
Personalised care planning working with individuals to identify needs, develop tailored care and support plans, build resilience, and increase engagement with local communities.
Community capacity building supporting community groups to be accessible and sustainable, and helping people establish new groups where needed, working collaboratively with partners across the PCN.

Patient support and care
Provide nonclinical advice and personalised support to patients.Undertake consultations via facetoface, telephone, video, econsultations, email and home visits.Use a holistic, personcentred approach based on what matters to the individual.Support patients with goal setting and action planning to improve wellbeing.Run communitybased dropin sessions to increase accessibility.Provide personalised support to individuals, families and carers to help them take control of their health, live independently and improve wellbeing.Take a holistic approach informed by the persons priorities and wider determinants of health.
Case management and MDT working
Take referrals from GPs, practice teams and a wide range of clinical and nonclinical MDT staff.Accept referrals from PCN Core Network PracticesManage and prioritise a caseload independently, responding to urgent needs where required.Work with individuals to coproduce simple personalised care and support plans, or reconnect them to appropriate community groups and statutory services.Participate in MDT meetings to support coordinated care.Escalate concerns appropriately when needs fall outside the scope of social prescribing.
Community development and partnership working
Work collaboratively with local partners to identify community assets including VCSE organisations.
Support community groups to receive social prescribing referrals, ensuring they are welcoming, inclusive and able to build confidence, skills and social connection.Help local groups become sustainable, increase capacity and maintain basic safeguarding processes.Identify gaps in community provision and contribute to developing new groups, networks and opportunities.Recording and IT
