Our Community team work in patients homes, linking closely with the GP practices in Mid Hampshire, covering week days only at present. Our community teams are based in Winchester City, Colden Common and Andover. This post will cover either Andover or Winchester and the surrounding rural areas providing rehabilitation for both integrated intermediate care and long term conditions.

We are a busy, friendly, cohesive team, who would welcome the opportunity to work with a new member of staff willing to contribute to the on-going development of the services we provide. Our Rehabilitation assistants work primarily to implement treatment plans for patients as directed by the Occupational Therapist or Physiotherapist. Previous experience working in a care or rehabilitation setting is highly desirable.

We have 2 positions available - 1 full time (37.5 hours per week) based at Winchester/Colden Common and 1 part time (22.5 hours per week) at Andover War Memorial Hospital.

Main duties of the job

To support the work of the registered practitioners in the integrated care team working within the parameters of the NHS South Central Assistant/Associate Practitioner Code of Conduct.

Maintain excellent communication with patients, relatives and members of the team regarding all aspects of care, demonstrating a variety of communication skills in accordance with patient need.

Perform patient assessment (under supervision and after appropriate delegation from the registered practitioner). Assessments to include tissue viability, nutrition and falls.

Implement and evaluate individual care plans, utilising practical and theoretical knowledge under the supervision of a registered practitioner.

Undertake the daily review of patients health and social care needs as required against the care plan; take the appropriate action to achieve the planned aims.

To carry out environmental assessments and consider need for equipment, including ordering, fitting and ensuring safe usage.

Perform area specific clinical competencies to enhance the delivery of patient care as delegated by the registered practitioner.

To carry out rehabilitation programmes as agreed with a registered professional.

To organise, run and facilitate clinics or groups.
