Job summary
For this role you will be predominantly working with CKD inpatients and outpatients and you will also spend some time working within dialysis patients. The post offers a good variety of clinical work and an excellent opportunity to work in an established MDT. There are 3 experienced renal dietitians working within NHFT and you will be managed by the renal dietetic team lead.
The successful candidate will be based at Northampton General Hospital site within the dietetic teams based there and you will be asked to support the renal team at Northampton General Hospital as part of collaborative working across the University Hospitals of Northamptonshire group.
You will predominantly deliver a service to inpatients and outpatients who have chronic kidney, disease stages 4-5, across the county and will provide care for day case and outpatient dialysis patients at the Northampton dialysis unit. You will work closely with the multidisciplinary team on inpatient wards, and work with the Nephrology team with both inpatients and outpatients. The role with require face-to-face work and telephone clinics.
Main duties of the job
The role involves:
Telephone clinics for advanced kidney care, some clinics may be attended in person and off main site.
Providing care for dialysis patients and advanced kidney care patients who may require specialist nutrition advice
o Undertaking c assessment of patients with complex presentations, multipathologies within acute inpatient and outpatient settings and provide effective monitoring
o Formulating individual management and treatment plans for patients using advanced clinical reasoning and developing comprehensive discharge plans including advice on appropriate ACBS prescribable enteral feeds and supplements to be used. This includes secondary prevention
o Working collaboratively with the wider multi- disciplinary health care team to ensure delivery of coordinated care and to provide specialist advice and attend case conferences, network meetings as appropriate.
o Evaluating patients' progress and measuring outcomes and contribute to and implement service improvement work
o Using a range of verbal and non-verbal communication tools to communicate effectively with patients to implement treatment plans. This will include patients who have difficulty with understanding communication
o Providing advice and training to patients, carers, colleagues and other healthcare professionals to promote an understanding of the aims of dietetic treatment to ensure a consistent approach to patient care.
o Managing clinical risk within own caseload.
