Job summary
Poole North Primary Care Network (a network of 4 GP practices working together to support the care needs of 52,000 patients) is looking for an Advanced Nurse Practitioner to work from the Canford Heath Group Practice, a busy but friendly practice of 12,200 patients to cover its acute on the day patients and some visiting.
The practice already has in place Advance Nurse Practitioners and this role seeks to expand our on the day capacity by providing the highest quality of patient care, from anautonomous, prescribing practitioner working independently and in conjunction with other health care professionals to assess, diagnose and treat the conditions of patients attending within primary care.
The role brings with it regular supervision from a GP partner and support from our other network ANPs.
Hours of work
Monday 8am to 4.30 (with half hour unpaid lunch break)
Wednesday 8am to 1pm
Thursday 8am to 4.30 (with half hour unpaid lunch break)
Friday 1.30 to 6.30pm
26 hours per week
Main duties of the job
act as an autonomous practitioner working independently and in conjunction with other health care professionals to assess, diagnose and treat the conditions of patients attending within primary care. Provide expert professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence. Undertake clinical nursing practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to people accessing primary health care services. Assess diagnoses and treat patients in surgery who require acute medical attention and refer appropriately in line with local and national guidance. Admit patients to secondary care for acute medical need. Refers as appropriate to other members of the multidisciplinary team, secondary care including referral for x-ray in line with policy and appropriateness. Order blood and other pathology diagnostic testing via ICE. Work within all relevant practice policies and procedural guidelines e.g. infection control, chaperoning, risk management. Prescribes independently but in line with the prescribing formulary. See patients from the age of 3 up excluding patients who are pregnant, working within competencies but mostly seeing all acute patients apart from those with pregnancy/gynae related issues or mental health acute needs. Sign fit notes when required Undertake appropriate home visits if required.

Qualifications Essential
The post holder will be required to be a fully qualified and registered Advanced Nurse Practitioner and hold an Msc in Advanced Practice, and/or have a Digital Badge (or be willing to work towards one.)
