Job summary

Peritoneal Dialysis Nurse - Community & Outpatient Unit

We are seeking a passionate and experienced Peritoneal Dialysis Nurse to join our expanding team. This unique role offers the opportunity to work both in the community and the CAPD outpatient unit, delivering high-quality, patient-centred care to individuals managing Peritoneal Dialysis as a home-based therapy.

Key Responsibilities:

Provide home visits to support patients with dialysis-related concerns, offering care in the comfort and safety of their own environment.

Train patients and caregivers to independently manage their therapy, with ongoing post-training assessments and additional visits for vulnerable patients.

Assess patients for suitability for Peritoneal Dialysis and guide them in selecting the most appropriate therapy.

Participate in outpatient clinics held four times per week in collaboration with consultants, supporting patients attending routine appointments and addressing treatment issues.

Perform routine procedures following unit protocols, including exit site dressings, PETs, adequacy testing, and line changes.

Main duties of the job

To have responsibility for the following aspects of care and management:

The post holder has delegated continuing responsibility for the management of the clinical areas in the CAPD unit.

They will provide clinical leadership to the nursing team in the absence of the Unit Manager.

They will lead the nursing team in the provision of safe and effective clinical practice and enhance the patient's experience.

The post holder will ensure continuity, sharing of ideas and the settings of standards of care.

Information and feedback will be shared to empower patients and members of the multidisciplinary team.

The post holder must possess fundamental management and facilitation skill in order to coordinate and supervise the activity/management of the staff, promote multidisciplinary working and foster a supportive learning environment in the absence of the manager.

Person Specification

Qualifications Essential

Registered Nurse NMC Nursing Degree level 6 Renal Trained Nurse with Peritoneal dialysis experence

Desirable

Level 6 degree or above Mentorship/SSSA course Management Course

Essential Essential

Has full UK driving licence & willing to use own car Venepuncture & Cannulation Skilled in use of computer/keyboard

Desirable

Anaemia management Quality improvement & auditing experience
