Social Care Practitioner Shared Lives Derbyshire
We are seeking a Social Care Practitioner who is passionate about working within a CQCregistered service and who shares our belief that people are best supported within their own communities to live fulfilled, familycentred lives.
The Shared Lives Derbyshire Scheme enables and monitors arrangements for adults who meet Adult Care eligibility criteria. Our service currently supports mainly adults with a learning disability and/or autism to live with approved Shared Lives carers in the carers family home. We are also keen to extend this model of support to other adults who may benefit from communitybased, familyled care.
Main duties of the job
As part of the Shared Lives Team, you will support and guide Shared Lives carers to provide safe, highquality care within their own homes, with a strong emphasis on family life, community inclusion, and CQC compliance. Shared Lives arrangements may offer longterm support, short breaks, or day support, all with the persons wellbeing at the heart of the service.

Person Specification
Experience Essential
Direct experience of work with disabled and older people in a social care or health care setting
Desirable
Caring for or supporting others Working in a team setting formally or informally with other disciplines Devising and implementing care plans
Qualifications Essential
Level 3 Diploma in Adult Care equivalent or higher
Desirable
Level 4 Diploma in Adult Care equivalent or higher General academic qualifications
