Job summary
The Community Specialist Palliative Care Nurse will join an established Specialist Palliative Team which provides a 7-day service.
The post holder will be expected to work independently and as part of the wider healthcare team and carry out a comprehensive holistic assessment of need for patients, with a palliative diagnosis, using their clinical reasoning and expert knowledge to ensure the delivery of quality clinical outcomes working in close collaboration with other health and social care professionals.
The post holder will be expected to contribute to the wider palliative and end of life care agenda within the community in Luton including the Bedfordshire Palliative co-ordination Service, provide education across a range of professionals and ensure the team provides a flexible responsive service that meets the local needs within Luton.

Main duties of the job
Undertake holistic health assessments of patients with complex palliative and end of life care needs in community settings
To demonstrate required Nursing skills
To proactively plan, co-ordinate, deliver and evaluate the provision of a high-quality specialist palliative and end of life care within the community .
Make appropriate clinical decisions independently and seeking senior guidance where necessary in reviewing, monitoring and managing patients.
Assess plan and evaluate care for patients, their families and carers
Monitor patient needs and interventions and appropriately report to the identified key worker.
To provide education to patients, their families and health professionals.
Actively participate and contribute to the weekly multidisciplinary specialist palliative care team meeting .
Actively participate and contribute to other locality multidisciplinary team meetings as an expert palliative care/end of life professional
Provide patients and their families /carers with information
Work as part of a multidisciplinary teams acting as patient advocate for specialist palliative and end of life nursing advice and care. .

Undertake holistic health assessments of patients with complex palliative and end of life care needs in community settings
To demonstrate required Nursing skills e.g. syringe driver care, basic wound care, pressure area care, catheterisation within the professional sphere of confidence.
To proactively plan, co-ordinate, deliver and evaluate the provision of a high-quality specialist palliative and end of life care within the community and across the primary and secondary care interface ensuing onward referrals are made as appropriate.
Make appropriate clinical decisions independently and seeking senior guidance where necessary in reviewing, monitoring and managing patients.
Assess plan and evaluate care for patients, their families and carers
Monitor patient needs and interventions and appropriately report to the identified key worker.
To provide education to patients, their families and health professionals.
Actively participate and contribute to the weekly multidisciplinary specialist palliative care team meeting discussing individual palliative patients with the wider specialist palliative care multidisciplinary team and caseload management / reviews.
Actively participate and contribute to other locality multidisciplinary team meetings as an expert palliative care/end of life professional
Provide patients and their families /carers with information
Work as part of a multidisciplinary teams acting as patient advocate for specialist palliative and end of life nursing advice and care. They will be expected to work with a wide range of health and social care professionals who work within the acute and community setting.

Person Specification
Knowledge & Skills Essential
Excellent communication and interpersonal skills - written and verbal with evidence of basic counselling skills Ability to be flexible working across organisations and work with a range of professionals. Ability to work autonomously and as part of a team
Qualifications & Training Essential
Educated to a degree or evidence of working at level. To have Completed the V300 or willingness to undertake the V300 non- medical prescribing course. Palliative care/oncology post graduate course
Desirable
Leadership skills. Non-Medical Prescribing Psychological skills training
Experience Essential
Significant and recent clinical knowledge of palliative and end of life care. Audit /evaluation of service/project Evidence of teaching/training experience.
Desirable
Community Nursing experience. Specialist palliative care knowledge or experience
