Care Coordinators play a key role within a Primary Care Network (PCN) to support patients to access the care and services they need. As part of the PCN team, Care Coordinators support proactive healthcare by contacting patients and helping them to engage with important preventative services such as cancer screening programmes, immunisations and chronic disease monitoring. Providing coordination and navigation of care and support across health and care services.

The role focuses on proactive patient outreach and coordination of care, helping patients to navigate services across General Practice, community health services and the voluntary sector.

This role primarily involves contacting patients to support proactive care initiatives and follow-up activity. The role is not clinical and does not involve undertaking general reception duties.

Care Coordinators help patients access preventative care, overcome barriers to healthcare and support the PCN to improve health outcomes across our population.

The role requires someone who enjoys speaking with people, is highly organised, has strong telephone skills and excellent spoken and written English.

The successful candidate will be confident using and learning different IT systems.

Main duties of the job

At Central and West Warrington PCN, Care Coordinators play a key role in supporting GP practices, patients and the wider Primary Care Network. This varied and rewarding role focuses on proactive patient engagement, improving access to services, supporting prevention initiatives and helping ensure patients receive the right support at the right time.

The role involves regular communication with patients and professionals, with much of the work centred around telephone contact, care coordination and administrative support activities. Care Coordinators work collaboratively across practices to support service delivery and improve patient outcomes.

Examples of duties include:

Proactive patient outreach, including contacting patients to arrange reviews, appointments, blood tests and healthcare interventionsSupporting prevention initiatives and population health programmesProviding digital support and promoting digital inclusionSupporting vaccination and seasonal health programmesCoordinating care pathways and signposting patients to appropriate servicesWorking as part of multidisciplinary teamsMaintaining accurate patient recordsSupporting service improvement initiatives

Care Coordinators play an important role within the Primary Care Network (PCN), supporting patients to access the care, services and support they need while helping practices deliver proactive, preventative healthcare. Working as part of the wider PCN team, Care Coordinators contribute to improving patient outcomes through patient engagement, care coordination and population health initiatives.

This is a varied and rewarding non-clinical role that focuses on supporting patients to navigate health and care services and helping ensure people receive the right support at the right time. The role contributes to improving access to care, reducing health inequalities and encouraging patients to engage with services that can support them to stay well.

A significant part of the role involves proactive outreach and follow-up activity, with much of the day-to-day work involving communication with patients by telephone. This may include contacting patients to arrange or encourage engagement with cancer screening programmes, immunisations, long-term condition reviews, blood tests and other preventative healthcare interventions. The role requires confidence in communicating with a wide range of people and the ability to build rapport and engage patients in a positive and supportive way.

Care Coordinators work closely with GP practices and multidisciplinary teams to coordinate care and support patients to access services across General Practice, community health services and the voluntary sector. This may include identifying barriers to healthcare access, signposting patients to appropriate support and helping people access services that best meet their needs.

The postholder will use clinical systems and population health tools to identify patients, maintain accurate records and support proactive care initiatives. Care Coordinators may support activity across multiple practices within the Primary Care Network and will contribute to service development and quality improvement as services continue to evolve.

This role is not clinical and does not involve providing direct patient care, undertaking general reception duties or managing routine GP appointment requests. Instead, it is focused on coordination, communication and supporting patients to engage with healthcare services.

Qualifications Essential

GSCE grade A to C in English and Maths

Experience Essential

Experience of working with patients, service users or the public to support access to services, interventions or support programmes Experience of working effectively as part of a multidisciplinary team Experience of supporting individuals, families or carers in a health, social care, community or voluntary setting

Desirable

Experience working in a Care Coordinator, healthcare, primary care, social care or community-based role Experience using clinical systems or databases within a healthcare setting Experience of proactive patient engagement or recall/outreach work

Knowledge and skills Essential

Friendly, professional and approachable manner with strong interpersonal skills Ability to listen, show empathy and communicate sensitively with a wide range of patients Excellent verbal and written communication skills in English, with the ability to communicate clearly and confidently with patients and colleagues, including by telephone, and accurately record information Confident telephone manner with experience of regular patient contact Strong organisational and time management skills, with the ability to prioritise and manage a structured workload independently Ability to work effectively both independently and as part of a multidisciplinary team across multiple practices Ability to build positive working relationships with patients, colleagues and partner organisations Competent IT skills, including Microsoft Office and use of digital / clinical systems Understanding of confidentiality, GDPR and safeguarding principles Ability to recognise when patients require escalation or referral to other services Commitment to equality, diversity and reducing health inequalities Emotional resilience and ability to work in a busy, fast-paced environment Understanding of personalised care approaches and the wider determinants of health
