Job summary
We are recruiting a band 6 nurse to work as part of our In-Reach Team based at Parklands House, Basingstoke. The team is part of the larger Older Persons Community Mental Health Team, and is specialist in working with people in care homes. The successful candidate will have a wealth of experience, skill and knowledge in mental health in older people, especially dementia.
The In-Reach Team aims to provide specialist assessment and support to people with complex cognitive impairment, and or other mental health problems. We work towards maintaining a safe and stable environment for the patient by supporting the care home staff with providing person centred care.
Working with our partners, including families and carers, Adult Social Health and Care, acute hospitals is essential. The right candidate has excellent communication skills, determination to advocate on the patients behalf. They will be able to manage a caseload safely, and discuss clinical assessments with the team. They will have sound clinical skills, be able to coordinate complex care, have good administration and IT skills, as well as good report writing, organisational skills and safeguarding.
Applicants must have a driving licence suitable for use in the United Kingdom and access to a vehicle for work purposes. Our working hours are Monday to Friday 9:00 to 17:00.
Main duties of the job
The Senior Community Mental Health Practitioner will work as a core member of the In-Reach team to co-ordinate the care of a complex caseload of service users. They will be supervising a Community HCSW.
Working under the supervision of the Clinical Manager, the practitioner will formulate care and risk management plans in accordance with identified need and developed in collaboration with the service user, care home staff and where appropriate their relative.
You will actively consider and participate in team development.
