Job summary

We are looking to recruit a Patient Administration Care Co-Ordinator to join our team at Abbey House Medical Practice.

The focus of this role is to work across all administrative teams with other primary care professionals within the Practice to provide patients with administrative support and timely access to services.

This is a permanent full-time position - 37.5 hours per week Monday to Friday, working between 10.30-18.30.

Main duties of the job

We are seeking an individual who is self-motivated and enthusiastic, approachable, compassionate, organised and must be able to work as part of a team. Previous experience of working in a people-centered environment is essential.

The successful candidate must demonstrate good communication skills face to face, over the telephone and written down plus be able to actively listen to both patients and colleagues.

A good knowledge of computers is essential. Previous use of SystmOne would be an advantage however full training will be given.

Job description Job responsibilities

Help patients to manage their administration needs through answering queries, making, and managing appointments, as well as appropriate signposting within and outside of the practice.Provide co-ordination and navigation for people and their carers, across health and care services working closely with the Secretarial Team, Patient Support Team and other Care Co-Ordinators.Supporting patients to access their medical records, granting online access and guiding patients on using online services, such as the NHS App.Understanding a patients level of knowledge, to ensure they have good quality written or verbal information to help them make shared decisions about their care, health and wellbeing.Make displays for noticeboards and ensure patients have access to up-to-date information available in the waiting areas to promote services and support their health and wellbeing; keeping leaflets and forms replenished at both sites.Updating and refreshing patient information forms and templates as needed and storing on TeamNet, as well as replenishing stocks of patients leaflets/forms in reception areas at both sites.Ensure patient and staff pigeonholes are checked weekly, removing items over 1 month old and following up as needed; to include contacting patients if necessary.Opening, distribution and processing of incoming post and e-mail and clinical correspondence; actioning/coding as necessary and/or forwarding onto the most appropriate team/clinician for further action.Completion of basic non-opinion forms and core elements of forms for review by the GP for signing.Handling of Subject Access Requests and Solicitor Requests.Raising invoices when required for medical services provided to patients.Administration and computerised entries with regards to the entire patient registration and deduction pathway.Filing patients records and retrieving written patient records as requested.General administration support to all teams; to include outgoing post and photocopying.Provide cover for other administrative teams in the Practice when required/directed by the Management Team.Act as a Chaperone for patients when requested.Where appropriate refer people back to other teams and healthcare professionals.To work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures whilst managing their own workload to deliver the practice priorities.Ensure that all activities are recorded, monitored and evaluated.Attend all meetings as required.Work in accordance with practice policies, ensuring that all work and procedures are undertaken in accordance with issued guidelines.Attend any training courses and supervision sessions as required.Promote the professional image and services offered by the surgery.Any other duties that may be required and agreed by the Doctors and Management Team

Qualifications Essential

Safeguarding certification, or willingness to complete Training as set out by the Personalised Care Institute, or willingness to complete
