Job summary

An exciting opportunity has arisen for an experienced, forward thinking and enthusiastic clinical pharmacist to join our CDDFT orthopaedic team based at Darlington Memorial Hospital.

Our pharmacy department is dedicated to delivering high-quality, integrated care, with a strong emphasis on enabling our clinical pharmacy staff to spend the majority of their time in direct patient-facing roles.

In this role, you will be a key member of the orthopaedic multidisciplinary team (MDT), with the opportunity to shape and develop the clinical pharmacy service across both acute and elective surgical pathways. As an independent prescriber, you will have a strong interest in perioperative care and proactively support pre-assessment services and enhanced recovery pathways to drive improved patient outcomes.

The role may require cross-site working, including attendance at Bishop Auckland Hospital, to support delivery of key service objectives.

Main duties of the job

Should you be successful, you will work closely with the consultants and wider MDT to deliver the trust values of safe, compassionate and joined up care. Our mission is 'right first time, every time'.

You will demonstrate leadership in the orthopaedic speciality and provide specialist advice on medicine use. This will include professional collaboration to contribute to safe peri-operative care and to support the team with evidence-based practice and pharmaceutical issues.

You will build on well established relationships between pharmacy and the orthopaedic team. A key part of the role will be to deliver the governance agenda, teaching, quality improvement, service development and sustainability. We will support the successful candidate with mentoring and development opportunities including practicing as an independent prescriber, advanced/ consultant portfolio development and other further education opportunities.

You will provide leadership to other pharmacy staff and will support the Lead Pharmacists in moving the trust's pharmacy strategy forward. You need to be able to work autonomously, whilst motivating others and being a team player.

Key responsibilities:

To facilitate a 7 day pharmacy service, all pharmacists contribute to an on call, late night and weekend rota.

Lead and manage a team of pharmacy staff and be responsible for managing the delivery of safe, effective and efficient pharmacy and medicines management services to a defined group of patients and service users.

Provide and support the delivery and development of pharmacy and medicines management services within the trust, including involvement in research and audit.

Work closely with the other Trust specialist pharmacists to develop the service across all trust sites. This will include representing the directorate at speciality meetings, providing training to pharmacy staff, providing specialist advice and expertise to all members of the directorate, the Trust and the wider health economy.

Maintain and develop a portfolio of personal clinical activity within the trust at a level commensurate with a senior pharmacist.

Employ models of practice that recognises the needs of patients and carers whilst maintaining practice that is embedded in the principles of risk minimisation and patient safety

Supervise, support and develop other pharmacy staff, in particular junior pharmacists, trainee pharmacists, and other Trust personnel as required

Monitor the practice of other healthcare professionals, including medical staff, to ensure that prescribing and medicines use is safe and effective for patients. This would include the resolution of local prescribing issues and contribution to the education and training of clinical and non-clinical staff to promote safe and effective medicines use within the trust.

Work closely with patients and healthcare professionals including consultant staff and Care Group leads to ensure services are developed and delivered in a manner that is consistent with the priorities and needs of patients and other service providers.

Liaise with other pharmacy teams, healthcare professionals and agencies both within and external to the Trust to consider medicine management issues across boundaries

Influence local practice to ensure safe medication use, and to ensure that their own practice and the practice of others effectively acknowledge good prescribing practice, patient safety and risk minimisation.

Act as a member of the site based senior pharmacy management team, contributing to strategy, policy development, operational delivery and financial planning

Person Specification

Statutory Registration Essential

Registered member of the General Pharmaceutical Council (GPhC)

Desirable

Membership of a pharmacy professional body e.g. RPS, UKCPA, BOPA, NPPG.

Qualifications Essential

Masters Degree in Pharmacy or equivalent. Relevant post graduate clinical qualification (to at least diploma level) or portfolio evidence of attainment to the same standard. Evidence of CPD. Pharmacist Independent Prescriber or currently working towards.

Desirable

Supervision, leadership or management training. Evidence of an up to date portfolio of personal professional activity. Additional training or qualification relevant to the post eg. PGCert.

Special Requirements Essential

Able to work with colleagues and to influence the practice and behaviours of others in a positive way. Motivated to develop personally and professionally through self-audit, reflection and learning. Good oral and written communication skills. Able to travel independently across Trust sites to meet the requirements of the post.

Experience Essential

Extensive experience of providing pharmacy services in a clinical setting at an advanced level. Regular experience of day-to-day registered pharmacy staff supervision. Experience of using computer software to produce reports or presentations to healthcare professionals. Involvement in development of clinical guidelines or protocols. Participation in the delivery of education and training in a clinical setting. Experience of providing clinical supervision to pharmacists.

Desirable

Relevant experience for this role. Experience of supporting staff through HR processes. Experience of providing directorate pharmacist services eg. drug reviews, specialist drug information. Experience of service development, research or quality improvement. Experience of presenting an audit or research at a conference. Experience of being a Prescribing or Educational supervisor to Foundation pharmacist trainees. Experience of teaching healthcare professionals in a classroom setting.

Special Skills & Knowledge Essential

Able to manage, monitor, advise and review medicines reconciliation for patients. Demonstrates accountability for delivering professional expertise and direct service provision as an individual. Recognises personal limitations and refers to more appropriate colleague(s) when necessary. Able to motivate self to achieve goals. Able to improve quality within limitations of service. Demonstrates use of appropriate communication to gain the co-operation of relevant stakeholders (including patients, senior and peer colleagues, and other professionals where possible). Able to plan and prioritise a pharmacy teams day-to-day clinical service which is a complex and changing environment. Demonstrates a good understanding of pharmacys role in governance processes. Demonstrates a working knowledge of evidence-based medicines information resources and the ability to interpret the information.

Able to demonstrate an area of expertise within a field of pharmacy practice. Able to motivate individuals and/or the team. Able to successfully manage a project at team and/or service level
