Ealing Short Term Rehabilitation - Band 7 Community Occupational therapist

We have exciting opportunities for a Permanent Band 7 Occupational therapist to join our specialist multidisciplinary community rehabilitation service.
Please note that our service has 2 bases and interviews will be held in person at our office in Level 10 South Ealing Hospital, London or EDTC, Southall UB1 2SH.
Main duties of the job
The Ealing Short Term Rehabilitation (STR) team provides therapy-led rehabilitation to help patients improve their abilities and achieve theirrehabilitation goals, through therapy led care. We focus on achieving goals and enabling people to regain independence and social integrationeffectively and efficiently after discharge from acute hospitals and community services in the borough. Therapists and rehabilitation assistantsoffer support for a period of up to six weeks, usually less if goals are achieved or care can be delivered in a different way. Our team includesPhysiotherapists, Occupational Therapists, Rehabilitation Assistants. The team provide a service in a variety of community locations but mostly inthe patients' own home.
The post holder will contribute to the Occupational therapy provision within the STR team and will deliver skilled OT care to patients in theirown home, based on a thorough holistic assessment of their clinical and personal needs and the agreement of an individualised rehabilitation careplan to the standards set by RCOT and HCPC.The post holder will participate in service-wide seven day working on a rota basis.

To undertake highly specialist assessment of patients, interpreting complex findings, to enable an appropriate treatment plan to be formulated, delivered and progressed for a wide range of complex conditions, using highly developed clinical reasoning and skilful therapeutic handling developed from specialist knowledge and using extensive practical experience to guide the process.
To assess, prescribe, issue, and instruct patients in the safe use of a variety of equipment including a range of functional aids, manual handling aids and mobility equipment.
To assess, diagnose and develop individual care/treatment plans for a range of conditions, some of which will be highly complex or will be for patients with complex clinical and social needs, and to be responsible for reassessing patients care as treatment progress and alter treatment programme as appropriate. To evaluate and develop treatment programmes.
To keep accurate up-to-date records on patient care and equipment provision, which comply with quality standards of Trust/Professional/Service policies, meet legal requirements and that can be shared with colleagues and carers when appropriate.
To advise and teach patients, carers and other professionals to participate in occupational therapy and intervention plans for the individuals.
To ensure a high standard of clinical and professional practice.
To support, supervise and complete appraisals for junior Occupational Therapists, Rehabilitation Assistants, students on placement and locum therapists.

Education/Qualifications Essential
Degree/ Diploma Occupational therapy Registration with HCPC as an Occupational Therapist

Membership and/or involvement in National special interest group Post-Graduate MSc degree Member of COT
Experience Essential
Previous experience of working in the UK or a similar health economy as an occupational therapist Previous general experience to include the following: o Older people/elderly rehab o Medicine & general surgery o Orthopaedics o Falls o Dementia o Long term conditions Experience of teaching juniors & assistants Experience of completion of audit and service developments
Desirable
Experience of working as an autonomous practitioner in a community setting Previous general experience to include the following: o Community working o Neurology o Pain Management o Group work
