About this role

Job summary This role helps people in our community by delivering safe, compassionate nursing care, making a real difference to their recovery and wellbeing. You'll work as part of an integrated community nursing team, supporting patients with a wide range of health needs, including complex and palliative care. The role is varied and rewarding, combining hands-on clinical care with teamwork, leadership and shared decision-making. You'll also support colleagues and students, helping to maintain high standards of care throughout each shift

Main duties of the job What you'll do:

Assess, plan, deliver and review nursing care for a group of patients Provide safe, sensitive care and communicate clearly with patients and families Coordinate the ward or unit during a shift when senior staff are not present Support and guide junior staff and learners in daily practice Maintain accurate records and follow clinical and safety procedures What we're looking for: A registered nurse with current professional registration Recent experience in a nursing role, with interest in community or palliative care Good communication skills and a calm, supportive approach Ability to work independently and as part of a team Willingness to learn, develop and support others We welcome applications from people of all backgrounds and experiences, and we value the different perspectives that help us deliver fair, respectful and inclusive care for our communities.

Person Specification

Qualifications Essential

RGN / RN / Diploma Studies Be willing to undertake further training and development

Desirable

Completion of post qualification preceptorship

Knowledge and Experience Essential

To have recently worked within a nursing capacity Demonstrate an understanding of the MDT

Desirable

Community Hospital experience Demonstrate an interest in palliative care Demonstrate an understanding of evidence based practice Have an interest in audit and maintaining standards
