Job summary
Providing high quality, holistic, family centred, nursing care for children in the community setting who meet the criteria for referral. The post holder will be an effective member of the Community Children's Nursing team. The post holder will provide skilled nursing care, working in partnership with organisations and child, patients, carers and families. The post holder will work independently, with supervision, within the community in line with local and national policies. The post may also support the special schools and involve working across sites during normal working week
Main duties of the job
The post holder will carry out clinical visits to family homes and educational settings providing care which prevent hospital admissions. They will advise families and allied health professionals with queries and have good experience in clinical skills.
They will be responsible for a caseload of patients but also be reactive to day to day requests from all patients who require intervention that day.
They will be part of a busy team communicating and working together to provide good quality care in the community.

To work in partnership with families and the Multi-disciplinary team to provide a high standard of care in appropriate settings to maximise life opportunities for the child and young person.
Demonstrate knowledge, skills, competency and innovative practice in
paediatric nursing Care,
Demonstrate continual evaluation of clinical practice and recommend
changes
Provide and maintain a high standard of skilled nursing care /skills for
children with additional health needs. This will include-
Taking bloods, repassing nasogastric tubes and gastrostomies, recording oxygen saturations, proving and giving rescue medication, providing suction, tracheostomy care, injections, dressings, pressure area care and assessments
Assist in the completion of detailed holistic assessments of children and
families, while giving consideration to cultural differences.
Clinically access children in homes/schools as appropriate and provide
support/information and the necessary advice or care to maintain the safety
of the child. This may at times evolve escalating and calling ambulances
Assist in the completion of Education and Health Care Plans
To establish or maintain relationships with the child and family when
relevant whilst the child is an inpatient in hospital to help facilitate smooth
discharge.
Provide training and support to other areas of the community i.e school and
nursery staff
Completing accurate and timely recordings of involvement with the child
and family and reporting any changes affecting the child and family
appropriately.
Working in conjunction with other agencies, ensure the appropriate
planning for the transition of care takes place in a timely fashion and meet
the on going needs of the child/young person and family.
Develop and maintain effective communication networks with professionals
from other agencies and voluntary groups.
To support in the development of integrated care pathways.
With supervision, post holder will take referrals as appropriate within tht
agreed scope of the role, as per the pathways and refer children and
families to other specialists and generic services as required
Be accessible and available to key stakeholders in order to ensure advice
is obtained ina timely manner.
Work in conjunction wither other nurses in the team and provide cover as
required depending on scope of practice
Maintain knowledge and competence in computer skills, in order to access
drives, databases, email and approved internet resources and critically
appraise sources of information and their evidence base.
To maintain clinical skills and practice in order to offer support and direction
to clinical teams and other multi disciplinary teams.
To provide training for parents/carers/schools/nurseries about complex
health needs and support services available. This may include child
specific training and will be in conjunction with band 6 members of the
team.
