Job summary
Are you an experienced and motivated nurse passionate about delivering outstanding community-based care?
An exciting opportunity has arisen to join the Frailty Virtual Ward, an innovative partnership between Leicestershire Partnership NHS Trust, University Hospitals of Leicester and Adult Social Care. The service provides a safe alternative to hospital admission for people with moderate to severe frailty, delivering high-quality care in patients' own homes through a combination of clinical assessment, face-to-face interventions and remote monitoring.

Working closely with Advanced Clinical Practitioners, Geriatricians, Allied Health Professionals, Social Care colleagues and the wider multidisciplinary team, you will help deliver responsive, patient-centred care while reducing avoidable hospital admissions. You will also contribute to service development and quality improvement within this evolving model of care.
The service operates 08:00-18:00, including weekends and bank holidays on a rota basis.
