Job summary

We are seeking enthusiastic Senior Community Practitioners (Nurses) to join the Nottinghamshire UEC services. We are recruiting two posts

Our UEC services support discharges from acute hospitals and prevent admission. We deliver hospital-level care at home using remote monitoring, digital tools and multi-disciplinary community support. Our services take a rehabilitative approach to provide an individual with an opportunity to thrive in their own environment. Independence and self management of health conditions is our primary goal. Our service is part of a system-wide collaborative across Nottinghamshire Healthcare, NUH, Sherwood Forest Hospitals, and Doncaster & Bassetlaw Teaching Hospitals , ensuring seamless care across acute and community settings.

We support pathways including frailty, respiratory and admission avoidance and Pathway 1 - Discharge to Assess . As a Band 6 Senior Community Practitioner, you will provide timely assessments, person-centred care and advanced clinical decision-making in patients' homes, working both autonomously and within an established MDT.

This role offers opportunities to develop advanced assessment skills, contribute to innovative care models and play a key part in system transformation. We welcome applications from nurses passionate about acute community care, digital innovation and supporting people to recover safely at home.

Main duties of the job

Deliver timely, person-centred assessments and provide high-quality clinical care to patients in our UEC teams for up to 14 days.

Work autonomously and within the MDT (including Matrons, Nurses, AHPs and support staff) to prevent unnecessary hospital admissions and support safe early discharge.

Interpret remote monitoring data, undertake face-to-face clinical reviews, identify deterioration and initiate appropriate interventions or escalation.

Lead and coordinate safe, effective discharge planning and contribute to MDT discussions on treatment and risk.

Support junior staff through supervision, role-modelling and clinical guidance.

Maintain accurate documentation on SystmOne and uphold professional standards, governance, and patient safety.

Work flexibly across Discharge to Assess, frailty and admission avoidance pathways, following lone-worker and dynamic risk assessment procedures.

Job description Job responsibilities

We are looking to recruit a dynamic, flexible, and compassionate Senior Community Nurse - with a strong rehabilitation ethos, to join this new and exciting service within the localities of Nottinghamshire.

You will work within the UEC team and in collaboration with system partners as part of a supportive multi-disciplinary clinical team with a dedicated focus of delivering excellent patient centred care and innovation.

You must be able to work flexibly over 7 days including weekend and bank holidays.

You will be providing care to patients in a community setting, who require high quality holistic evidence-based care that is responsive, effective, and efficient in line with the virtual ward model.

Youpossess community nursing experience or acute care experience with a passion for learning, demonstrate excellent communication and interpersonal skills with the ability to be adaptable, reliable, resourceful, and approachable, evidencing high levels of personal and clinical credibility, being able to instantly establish effective working relationships in a range of work settings.

You have a thirst to use digital and remote technologies within your assessments.

You demonstrate high levels of care and compassion to promote independence, prevent hospital admission or admission to care, support early discharge from hospital and to help people to receive the care they require within the places they call home.

You enjoy working collaboratively across organisation boundaries, within a multidisciplinary framework, analyse and develop treatment plans whilst providing compassionate, realistic, and timely support to a diverse patient group and their families

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment.
