Introduction

Are you a Registered Nurse looking for an opportunity to develop your clinical skills, make a meaningful difference to people’s lives and take the next step in your nursing career?

As Castleholme Lodge’s Physical health Nurse, you’ll have the opportunity to play a key role in promoting and maintaining the physical health and wellbeing of adults living with complex mental health needs. You’ll work collaboratively with our multidisciplinary team and external healthcare professionals to ensure that physical healthcare is an integral part of every individual’s care.

If you’re passionate about physical healthcare, enjoy supporting and developing others, and want to make a real difference, we’d love to hear from you.

Where you will be working

Location: Castleholme Lodge, 4 Bryan Road, Edgerton, Huddersfield, HD2 2AH.

Castleholme Lodge is situated in Huddersfield, with excellent transport links to the surrounding areas. It is undergoing a complete refurbishment to provide a welcoming, high-quality environment for both service users and colleagues.

Castleholme Lodge is a 20-bed care home with nursing, providing support to adults with severe and enduring mental health difficulties. As part of a wider network of Elysium services across Yorkshire and the Midlands, you’ll benefit from opportunities to collaborate with colleagues, share best practice and learn from a wider clinical network.

What you will be doing

As our Physical Health Nurse, you’ll take a lead role in coordinating physical healthcare across the service, ensuring that individual needs are identified, monitored and addressed through effective, evidence-based care.

Your responsibilities will include

  • Developing, implementing and reviewing individual physical healthcare plans to ensure that service users’ needs are identified and supported through clear, effective interventions.
  • Coordinating physical healthcare assessments, investigations and treatments to promote and maintain optimum physical health and wellbeing.
  • Supporting colleagues to ensure routine and ad hoc clinical tests are completed as prescribed, with results accurately documented in care records.
  • Overseeing routine monitoring for long-term conditions, including asthma and diabetes, ensuring practice is consistent with current national guidance.
  • Ensuring ECGs are completed as prescribed by GPs and appropriately documented within care records.
  • Attending weekly ICR meetings to contribute to the development and review of individual physical healthcare plans and goals.
  • Co-chairing a monthly Healthy Living Group, promoting positive lifestyle choices through topics such as nutrition, weight management, smoking cessation and exercise.
  • Undertaking clinical audits and supporting emergency drills to promote safe, effective care and continuous service improvement.
  • Delivering educational workshops and teaching sessions to colleagues on physical healthcare, clinical best practice and relevant treatment approaches.
  • Supporting the timely delivery of or access to immunisations, including influenza and routine vaccinations, in line with relevant guidance and individual requirements.
  • Providing health promotion and healthcare advice to colleagues across a range of physical health topics.
  • Working collaboratively with service users, families and external professionals to encourage positive approaches to health and wellbeing.
  • Supporting the wider clinical team and providing clinical cover when required.

To be successful as our Physical Health Nurse, you’ll need:

  • Current and valid NMC registration as a Registered Nurse (evidence required).
  • A minimum of two years’ post-registration experience.
  • Experience in phlebotomy.
  • Experience of delivering training or supporting the development of others.
  • An understanding of mental health and eating disorders.
  • A passion for promoting physical health, improving outcomes and delivering high-quality, person-centred care.

Experience of working with older people would be desirable.

What you’ll get in return

At Elysium Healthcare, we recognise that our colleagues are central to the quality of care we provide. That’s why we offer a comprehensive benefits package designed to support your wellbeing, professional development and future.

Salary and annual leave

  • Annual salary of £39,281.
  • The equivalent of 33 days’ annual leave, including bank holidays, plus your birthday off.
  • The option to purchase additional annual leave during the annual selection window.

Your professional development matters. We want you to feel confident, supported and equipped to deliver excellent physical healthcare. You’ll have opportunities to develop your clinical knowledge, access relevant training and continuing professional development (CPD), share expertise with the multidisciplinary team and contribute to the ongoing development of physical healthcare across the service.

We’ll support you to identify learning opportunities that align with your role, professional interests and future career ambitions, helping you build your clinical confidence and continue to grow as a healthcare professional.
