About this role
Job summary
Following a recent service review, we are delighted to introduce the Keyworker role as part of our enhanced non-clinical offer. We will be recruiting for one position, aligned to a locality area:
o Boston
The core purpose of the service is to support young people, their families and carers to navigate health and social care systems, particularly where there is a risk of admission to a mental health inpatient setting.
You will hold a defined caseload and work in partnership with a dedicated Keyworker Support Worker, delegating tasks appropriately and ensuring that all interventions are delivered safely, effectively and in line with agreed care plans.
A central aspect of the role is to ensure that the voice of the young person--and that of their parents or carers--is heard, understood and meaningfully represented throughout their journey. This requires strong relationship-building, consistency, and the ability to empower families to confidently engage with complex systems, while promoting community-based support wherever possible.
You must have a lived or learnt experience of Autism and/or Learning Disabilities.
Due to the UKVI immigration changes we are no longer able to offer sponsorship for this role. Please provide details of the visa type and expiry date in your application. Failure to provide the required information will result in your application being rejected.
Main duties of the job

Act as a consistent point of contact, coordinating care and ensuring effective multi-agency working across health, education, social care and the voluntary sector.
Support families to navigate complex systems, providing clear information and enabling them to access the right support at the right time.
Build trusted relationships and ensure the voice, wishes and experiences of the young person and their family are heard and represented in all planning and decision-making.
Identify, assess and respond to risk and safeguarding concerns in line with policy, escalating where appropriate.
Work proactively to prevent hospital admissions and support safe discharge and successful transitions, including into adult services.
Signpost and refer to appropriate services and community resources, promoting independence and community-based support.
Monitor progress against agreed outcomes, contributing to personalised care planning and step-down arrangements.
Maintain accurate, timely records and contribute to service evaluation and improvement.
Support and guide a Keyworker Support Worker, ensuring delegated tasks are completed safely and effectively.
