About this role
Job summary
We have an exciting opportunity for a compassionate, organised and proactive Care Coordinator to join our team at Horizon Healthcare.
This is an important non-clinical role supporting patients who may have complex health and care needs to navigate services and receive coordinated, personalised support.
You will work closely with GPs, the wider practice team and colleagues across community healthcare, social care and the voluntary sector. You will manage a defined caseload, build relationships with patients and their families, coordinate care and follow-up, and help ensure that people receive the right support at the right time.
This would be an excellent opportunity for someone with experience in healthcare, social care, support services or a customer-focused environment who enjoys bringing people and services together and making a real difference to patients.
This is an opportunity to take on a role where you can have a tangible impact on patients who may find healthcare and other services difficult to navigate.
You will become part of a multidisciplinary environment where collaboration is central to how we work and where staff are encouraged to contribute to quality improvement and the development of our services. The practice supports learning and development, including mandatory training and, subject to approval, external development opportunities that enhance an individual's knowledge, skills and career development.

- Coordinate referrals, appointments and follow-up arrangements within the scope of the role.
- Support effective transitions between hospital, community and primary care services.
- Work closely with GPs, nurses, pharmacists, social prescribing link workers, health and wellbeing coaches and other professionals.
- Coordinate and provide support for multidisciplinary team meetings, including preparation, recording actions and following these up.
- Maintain accurate patient records, caseload information and activity data.
- Identify delays, gaps in care, safeguarding concerns or other risks and escalate these appropriately.
- Contribute to service improvement, audit and initiatives aimed at improving access and reducing health inequalities.
This is a non-clinical position, and the successful candidate will be expected to recognise the boundaries of the role and appropriately escalate clinical, safeguarding or other significant concerns.
