Qualifications
- High School Diploma or equivilent
- Three years of responsible full time clerical experience
- Excellnent Customer Service Skills
- Ability to make decisions in accordance with departmental policies and procedures
Duties and responsibilities
- Assist individuals serviced by addressing inquiries, resolving issues, and fulfilling requests across diverse communication channels.
- Answering and triaging internal and external telephone calls
- Pulling, filing, autiding, and maintainingi charts for the teams assigned.
- Closing team charts
- Co-facilitating team meetings minutes including huddles regarding team-based care, integrated healthcare, and other population health
- Review and share data reports and outcomes of team services
- Assist wtih the coordination of care for individuals served on the team
- Comelting vitals and documenting in E.H.R.
