About this role
Job summary
We are seeking a highly skilled and autonomous Advanced Nurse Practitioner (ANP) to join our team for up to 24 hours per week. This role is pivotal in providing high-quality, patient-centred care in a fast-paced, supportive general practice environment.
As an ANP, you will lead on delivering undifferentiated primary care to a varied patient population, working independently but with access to clinical support from the practice team. The successful candidate will demonstrate excellence in clinical assessment, decision-making, and prescribing, with a commitment to upholding the standards of care and safety expected by Coleridge Medical Centre.
An exciting opportunity to help shape frailty care:
Alongside our established in-house ANP service, we are developing an exciting new proactive frailty pathway, focused on early identification, comprehensive assessments, personalised care planning and multidisciplinary working to improve outcomes for our older and more vulnerable patients. This offers a fantastic opportunity for an ANP with an interest or experience in frailty to play a key role in shaping and developing the service from the outset. An interest in frailty is by no means essential, however, and we would equally welcome applications from enthusiastic and experienced ANPs looking for a rewarding, varied role within our general practice team.
