About this role
Job summary
This is an exciting opportunity to join Harrow Councils Public Health team as a Public Health Disease Prevention Nurse, playing a key role in improving the health and wellbeing of residents across the borough.
This role is ideally suited to a candidate with experience of community work like a district nurse, health visitor, school nurse, nurse for safeguarding who is looking for an exciting new challenge. This is a new role that you can shape and make your own. You will have a high degree of autonomy and the chance to have a life-changing impact on thousands of patients in Harrow. We carry out up to 8,000 NHS Health Checks a year.
Focussed on working through others, influencing, training and sharing best practice rather than direct patient care, you will support the development of skills, confidence and capacity within primary care to ensure Harrow patients receive a high quality NHS Health Check and lifestyle advice. Through this work, you will help reduce the risk of cardiovascular disease, prevent long-term conditions and encourage healthier lifestyles across Harrows communities. Main duties of the job
A typical day might include visiting a couple of the 31 GP practices in the borough, getting to know the staff, observing an NHS Health Check and any resolving problems and queries that you find. You will act as an important bridge between the local authority and primary care, feeding back common issues and opportunities identified through conversations with practices and helping to develop borough-wide approaches where appropriate.
As a specialist public health resource, you will lead the development, delivery and evaluation of evidence-based training, education and quality improvement initiatives. By using data, research and best practice, you will help drive improvements in service delivery, address health inequalities and ensure that programmes meet the diverse needs of Harrows population.
This rewarding role offers the opportunity to influence positive change across the local health and care system and make a lasting impact on population health. You will work collaboratively with a wide range of partners to promote culturally competent, compassionate and evidence-based approaches to behaviour change and disease prevention.
