About this role

Job summary

Cornwall Partnership NHS Foundation Trust is seeking an experienced Transformation Programme Manager, Neighbourhood Frailty, at Band 8a to coordinate the design and delivery of the 18 month Neighbourhood Health Frailty Initiative across Cornwall and the Isles of Scilly.

Working across all 16 Integrated Neighbourhood Teams, you will bring together primary care, community and acute services, councils, commissioners and voluntary, community and social enterprise partners to improve services for people living with frailty.

You will translate national and local priorities into practical delivery plans, lead programme teams and oversee governance, financial control and evaluation. Working with people living with frailty and their carers, you will ensure their involvement informs programme decisions.

We are looking for someone with extensive experience of delivering complex service change, strong programme management skills and the ability to build effective partnerships across organisational boundaries. You will bring knowledge of neighbourhood health, frailty and integrated care, alongside the analytical, communication and leadership skills needed to manage competing priorities and demonstrate results.

You will work in line with our CHOICE values: Compassion, Happiness, Ownership, Integrity, Connection and Equity.

Main duties of the job

Coordinate the neighbourhood frailty programme from scoping and design through delivery, evaluation and benefits realisation across all 16 Integrated Neighbourhood Teams. Develop delivery plans, milestones and governance arrangements. Manage interdependent workstreams, monitor progress, maintain programme records and escalate risks and issues to the Programme Board. Lead and support multidisciplinary programme teams, including line management, objective setting, coaching and development.

Build effective relationships with clinical, operational and system partners, resolve competing priorities and involve people living with frailty and carers in decision making. Develop costed business cases and maintain financial control of the £6.8 million non recurrent programme, monitoring expenditure, savings and value for money. Use population health, activity, performance and equity data to inform service redesign. Coordinate formal evaluation, agree outcome measures and report on quality, safety, equity, performance and value.

Ensure compliance with clinical safety, information governance, data protection, safeguarding and equality requirements. Present findings and recommendations to programme boards and senior leaders, sharing learning and best practice across partner organisations.

About us

We're an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly. We deliver community and hospital-based care to improve people's physical and mental health. We also provide specialist support to people with dementia or a learning disability.

We are a people organisation and people matter to us. As part of the team, you'll help support the health and wellbeing of the people who live and visit this beautiful part of the UK.

Over 5,000 people make up the Trust. This includes doctors, nurses, therapists, plus admin and support staff.

We work in people's homes, in community clinics and bases. Some staff work from one of our 10 community hospitals. Our aspiration is to have great people, provide great care, be a great place to work and a great partner.

Approximately 568,000 people live here. A third of people who live in Cornwall are supported by acute hospital services in Devon. As a result, we also work closely with our partners in Devon. In the summer, and during other holidays lots of people choose to visit the area. This increase the numbers of people who use out services.
