Job summary
We take referrals for medical , nursing and therapy crisis from the community and (e.g NWAS, GPS ) and SRFT departments, aiming to promote early discharge from hospital and avoid unnecessary hospital admissions; by implementing clinical treatment/care plans in the patient's own homes in the Salford area.
The team manages acutely unwell patients at home using a variety of pathways e.g. IV antibiotics via cellulitis pathway , DVT pathway , heart failure pathway , short term antibiotic pathways,we are currently developing further pathways to manage patients in their own environment and prevent unnecessary admissions to hospital
This is expanding to include Hospital and Care at Home (H&C@H) virtual wards model of care working as part of a team utilising technology and new ways of working to provide a safe alternative to hospital admission and support people to be discharged from hospital earlier than may have been possible previously.The focus of this role is delivering high quality care in the community, holistic assessments, planning, implementing, and reviewing care provision across the Virtual Wards pathways of care. This is an exciting opportunity to shape services that will improve the experience of healthcare for the population of Salford and deliver an NHS national priority.
Main duties of the job
We are looking to recruit an experienced Nurse Practitioner to join our Community Urgent Response/Hospital and Care at Home (H&C@H) virtual wards / Home IV (OPAT) team to work with a multidisciplinary team of clinicians including Advanced practitioners Nurses, Allied Health Professionals and support staff to provide high quality comprehensive care to patients who require step down from hospital and provide clinical interventions in the home environment to prevent hospital admissions , Working closely with NWAS and GPs.
You will have current NMC Registration, preferably 2 years community experience and a clinically based nursing degree and car user is essential to this role.
As you are expected to drive for the role you must have UK driving license with access to a car with business insurance

Qualifications & Professional Registration Essential
Registered Nurse with current NMC registration (Part 1 Adult / General Level 1 or 2) Degree / Diploma in Nursing studies or equivalent
Desirable
V300 prescribing qualification ENB998/Preparation for Mentorship/ Evidence of supporting students
Knowledge/experience Essential
Substantial post registration experience Experience of working as part of an MDT

Ability to organise and prioritise own delegated workload Able to carry out basic observations and monitoring and use relevant equipment safely To utilise highly developed physical skills e.g., in preparing and giving IV injections, assembling surgical equipment, maintaining infusions Able to coordinate own workload ensuring high quality, safe and effective patient care Ability to travel to different geographical locations across the Trust footprint in line with service needs
