About this role

Job summary

Gillingham Medical Practice is looking for a highly motivated, experienced practitioner to join our Frailty Team, working 25-30 hours per week. This is an exciting opportunity to make a real difference to the lives of patients living with frailty and complex health needs.

Working alongside our existing Advanced Nurse Practitioner and GPs, you will play a pivotal role in delivering proactive, holistic and patient-centred care to individuals living in their own homes and in care home settings.

As a Frailty Practitioner, you will:

Undertake comprehensive assessments of patients with multiple co-morbidities and complex needs

Work autonomously to assess clinical presentations and initiate appropriate investigations

Deliver evidence-based, holistic treatment and care plans

Coordinate and optimise care across services

Refer patients to appropriate medical and specialist services when intervention is required

You will be a key clinical resource within the Practice, supporting high-quality frailty management and contributing to innovation and service development.

We are a well-established two-site practice serving over 13,000 patients across a rural community, with a dedicated traige hub at the heart of our service. Job description Job responsibilities

* To coordinate the delivery and development of the specialist service through the use of effective evidence based practice in line with agreed national standards for patient safety and monitoring.

* To be accountable for the planning and delivery of safe, effective and compassionate patient care in relation to dedicated pathways of care, working as an autonomous practitioner.

* To work autonomously, assessing individual patient needs, initiating investigations in line with evidence based treatment and care.

* Refer patients who require medical intervention to the appropriate services.

* Manage care at the interface for patients in their own homes and care homes, preventing avoidable admissions.

* To continuously improve clinical care standards in line with the clinical governance framework.

* To exercise accountability as set out in the NMC Code.

* Provide specialist clinical advice and support to the patients and clinical staff on the management and care of patients.

* To provide direct access for patients, either face to face or telephone consultation. Along with telephone triage where appropriate.

* To support GPs and ANP in care home ward rounds, to review patients and play a key role in optimising complex treatment regimes and preventing avoidable hospital admissions.

* To ensure effective communication internally and externally regarding patient care and resources.

* Ensure effective communication with patients, relatives and carers, taking into consideration level of understanding, culture and background.

* Escalate any concerns to the Clinical Supervisor.

* Encourage relatives to take an active interest in the welfare and care of the patient.

* Ensure all documentation and record keeping is accurate, legible and completed in a timely manner, in accordance to NMC and practice policy.

* Maintain effective links with community, intermediate services and practitioners, utilising their skills and knowledge.

* Actively participate in professional forums including Practice meetings, multi-professional meetings.

Person Specification

Qualifications Essential

Relevant Nursing/Health Degree Registered with NMC Enhanced skills accreditation Evidence of recent professional development in an up to date portfolio Full UK Driving License and Access to own car including business insurance

Desirable

Non Medical Prescribing Qualification Training and experience in specialist areas
