We are looking for a skilled, enthusiastic and experienced Clinical Pharmacist to join Haydock Medical Centre on a full-time basis. In this role, you will be instrumental in delivering high-quality, patient-centred pharmaceutical care, optimising medicines safely and effectively, and driving innovation in Primary Care.

Main duties of the job
This is a dynamic, patient-facing Clinical Pharmacist role, focused on delivering evidence-based pharmaceutical care in Primary Care. You will use your expert knowledge of medicines to support patients with specific disease areas and long-term conditions.
Key responsibilities include:
Clinically assessing, diagnosing, and prescribing (where qualified)
Optimising medicines and improving prescribing safety
Collaborating with a multidisciplinary team to enhance patient outcomes
Supporting medication reviews and structured medicines optimisation
Contributing to service development and innovative working practices
Working with Divisional Boards and the National Modality Business Support Team to deliver strategic priorities
