About this role

Job summary

We have an excellent opportunity for an experienced Health Care Assistant to join our growing organisation.

The post holder will work alongside all members of the nursing team and strictly in accordance with specific Practice guidelines and protocols, the healthcare assistant will assist the clinical team in the provision and delivery of prescribed programmes of patients care for our patients in the Chesterfield area.

This is a part-time permanent role - 24 hours per week.

PLEASE NOTE: This role does not meet the criteria for visa sponsorship.

Main duties of the job

To prioritise and organise your own case load across all three of our chesterfield sites, to complete annual long term health reviews, phlebotomy, blood pressure monitoring, smoking status, provide lifestyle advice and health promotion.

To deliver care in a variety of settings including patients own home, nursing and residential care settings.

Work under Patient Specific Directions (PSD) with recognised immunisation training when requested, in accordance to policies and procedures.

To prepare, maintain, restock and stock rotate clinical environments on a weekly basis, ensuring documentation is completed.

Ensure accurate and legible notes are kept for all consultations, and treatments are recorded in the patients notes.

To maintain display areas for health promotion and order materials when required.

The post holder will require an understanding, acceptance and adherence to the need for strict confidentiality and the ability to use their own judgement, acceptance of own capabilities and awareness of own limitations.

The post holder may require working alone and determining their own workload priorities.

Excellent communication skills and basic computer skills are required.

Person Specification

Qualifications & Training Essential

Education to GCSE standard (or equivalent), to include Maths and English. NVQ level 3 in Primary Care.

Desirable

Health Related Training and First Aid.

Experience Essential

Experience of dealing with the general public. Understands the need for confidentiality. Understands and is committed to a caring approach to patients, relatives and carers. Experience in a NHS/healthcare setting

Desirable

Experience working within a Primary care environment. Experience of using clinical computer systems.

Skills & Knowledge Essential

To be able to demonstrate a caring approach to patients and relatives and to ensure dignity and respect is maintained at all times. To be able to communicate effectively with excellent communication skills. To be able to work under your own initiative within the boundaries of this role. To be able to prioritise your own workload effectively. To demonstrate awareness of working as part of a team and be able to play an active role within this.

Desirable

Knowledge of SystmOne. A basic understanding of General Practice. Basic nursing skills.
