Palliative Care Nurse
Band 6
Full Time 37.5hrs
We are seeking a motivated, dynamic, and passionate Band 6 Community Palliative Care Nurse to work within our South Bedfordshire Team, based in Dunstable. There is an expectation that staff will cover across the the localities in Bedfordshire if required.
This is an excellent opportunity to develop your knowledge and skills within the specialist palliative care setting. The team provide symptom management, psychological support and advanced care planning, enabling patients to remain at home in their preferred place of care with the right support. You will have the opportunity to develop skills in holistic assessment and clinical decision making, working within a highly specialist area of practice.

Main duties of the job
The post holder will diagnose and treat complex health care needs or refer to an appropriate specialist utilizing expert knowledge, history taking and examination skills.
Provide emotional support to patients and carers using sensitive communication skills often undertaken in difficult family situations or circumstances.
Initiating conversations regarding advanced care planning and decision making.
Make appropriate referrals to other services who can help provide the required care and support. Liasing with community health care providers and the wider MDT to ensure seamless palliative care support.

Youmust have a driving license and have access to a vehicle in order to carry out your duties. You will be required to work full time (37.5hrs) Monday to Friday between the hours of 8am - 6pm.
