Job summary

An opportunity for a Senior Nurse to join the Community Assessment Unit (CAU) at Gosport War Memorial Hospital and the Urgent Care Response (UCR) team in the community.

The post is a 30 hour-per-week substantive post covering the Fareham and Gosport Area, with the understanding that support will be provided to Petersfield CAU.

You will be working within the outpatient CAU clinic setting at Gosport War Memorial Hospital and in the community setting where you will be responding to the urgent care needs of service users in their own homes.

This is an excellent opportunity if you enjoy having varied work experience and want to broaden your knowledge and skills in both a clinic and community setting. This role is critical to preventing hospital admissions as part of the Ageing Well agenda and monitoring patients in their own home as part of the UCR virtual ward.

As the role will be delivered in patients' homes and clinic-based settings, you will be working closely with all members of the multidisciplinary team. We are looking for an enthusiastic, caring and motivated professional with excellent clinical and communication skills, who can work autonomously within the scope of practice as a registered practitioner.

This position involves a proportionate amount of time driving when carrying out UCR visits, therefore you will need to be a car owner and have access to a vehicle for work with the correct level of insurance.

Main duties of the job

* To work independently within the team and to be responsible for the Community Assessment Unit caseload and prioritising effectively, to ensure timely assessment and discharge.

* Carry out all aspects of patient care, treatments and procedures within agreed policies and procedures of the Trust.

* Take on a specific area of interest/project/link role and act as a resource for the team to develop.

* Deputise and support the Clinical Manager, ensuring smooth and safe running of the Unit.

* Participate in the appraisal system.

* Address poor performance as it occurs and manage and report any serious incidents of misconduct.

* Effectively manage staff absence according to trust policy.

* Ensure newly appointed staff are fully inducted and documentation completed.

* Ensure effective and safe skill mix when completing the three-monthly electronic roster.

* Assist in the first line treatment and referral of patients with complex medical conditions under the frailty model.

* Give and receive regular reports on the progress of patients, immediately acting on any changes in the patients' condition and reporting them to the relevant health care professional.

* Work with medical staff and other members of the health care team to ensure that treatments are carried out within agreed policies.

* Receive patients and their relatives in a calm, courteous and reassuring manner, participate in providing information and support.

Job description Job responsibilities

Upholding all standards as identified by the NMC including the Code of Professional conduct, Documentation and Medicines Management or equivalent according to job title. The post holder will work as an active member of the Community Frailty Team for the Fareham and Gosport Community Assessment Unit and provide a high quality, efficient and effective service to patients.

To assist in the delivery of effective and timely assessments for patients who are within the speciality of frailty. To work alongside Consultant Geriatricians, GPs with Specialist Interest, Nurses, Physiotherapists and Occupational Therapists to provide a high standard of individualised nursing care in accordance with agreed policies and utilising an evidence-based approach. To be responsible for the assessment of patients' needs and the development, implementation, and evaluation of planned treatment.

The post holder will be a Registered clinician and will continue to develop their role through the acquisition of further skills, knowledge, and competencies to be determined by the needs of the patients, the service, and the team.

The primary role of the Senior Community Nurse is to provide strong holistic leadership to the team and support the Community Matron. To include day to day leadership responsibilities, development and creative leadership, responsive leadership, enabling the team to achieve a 'well led' culture.

The community teams in conjunction with a wide range of clinical colleagues and specifically, GP's, Practice Nurses and Social Care professionals, will lead and facilitate a patient or client focused, co-ordinated case management approach across primary and secondary care for people who are most vulnerable to, and at high risk of repeat admissions to hospital

The candidate will participate in and influence efforts across health and social services to shape multi-disciplinary pathways designed to support patient choice, improve quality of life, promote self-management and assure early intervention through the proactive provision of care in or as close to the patients own home as possible. The candidate will work across the caseload and the single point of access or (equivalent), using their clinical skills to identify the needs of patients and the correct services to liaise with

Person Specification

Qualifications Essential

BSc Nursing or RN currently registered with NMC. Post registration qualification or University Degree Teaching and assessing qualification ENB 998 or equivalent/mentorship qualification - PETALS Management/Leadership Qualification/development programme Hold a valid driving licence and have access to a car with appropriate business insurance

Desirable

Masters degree or equivalent experience gained by undertaking on-going personal development and training. History Taking and Physical Assessment Course or a willingness to work towards this in the future.
