About this role

Job summary

This is a Fixed Term/Secondment vacancy until 28/03/2027.

The vacancy sits within the Carmarthen GP cluster locality supporting people with chronic respiratory disease within their homes and community clinics. They will require specialist respiratory assessment skills and work autonomously but alongside partners from Primary, Community and Secondary Care to improve outcomes for those living with long term respiratory conditions.

Main responsibilities would include managing a clinical caseload of people living with chronic lung conditions. They'd be required to prioritise their workload, delegate work to other and supervise other staff.

Main duties of the job

The potholder will require specialist respiratory knowledge to support in the management of conditions such as COPD, ILD, bronchiectasis, home oxygen and home ventilation. We'd be expecting the postholder to have a high level of communication skills. Experience and skills in supporting those coming to the later stages of their life would be advantageous as would previous community working.

Being able to commute across a wide geographical area is essential.

They will be supported by their community respiratory colleagues and the wider respiratory team including general respiratory, bronchiectasis and respiratory failure virtual MDTs. They will have the opportunity to support Pulmonary Rehabilitation Programmes, an active respiratory research programme as well as service development and improvement. They would need to supervise, support and work alongside a Health Care Support Worker.

Person Specification

Qualifications and Knowledge Essential

Registered Nurse NMC registration Master's level qualification or equivalent experience Qualifications in asthma and COPD Teaching qualification Qualification in Non-Medical Prescribing or willing to undertake. Leadership and management accredited course Evidence of content of key current national or international guidelines.

Desirable

Other respiratory diploma or degree level qualifications ARTP, or similar accredited, Practical and Interpretation Spirometry certificate

Experience Essential

Evidence of working in an area with COPD, asthma, bronchiectasis, Motor neurone disease patients. Experience of providing clinical supervision. Experience of monitoring and evaluating services. Experience of teaching other disciplines. Experience of presenting to a range of audiences. Evidence of the use of asthma action plans. Experience of ordering and interpreting investigations/tests Managing patients needing acute CPAP Managing patients using acute and home based non invasive ventilation

Desirable

Experience of leading a clinical team. Involvement in change management. Evidence of assisting in the planning and developing of services. Published work. Experience of undertaking audits, writing reports and presenting audit outcomes to other health professionals
