Job summary

We are currently seeking a dedicated band 6 key worker to join us.

This role presents an innovative opportunity to positively impact the lives of individuals facing complex and enduring mental health challenges and stigma. Through innovative approaches like Dialog+, we collaborate closely with service users, families and carers, understanding their unique experiences, identifying personal goals, and providing tailored support to empower them on their journey to wellness.

As a valued member of our team, you will have the autonomy to deliver high-quality care while also benefiting from collaborative efforts. Your responsibilities will primarily include a holding a caseload of service users , and supporting along with your occupational therapy, admin, psychology, psychiatry and support workers colleagues to support their mental health journey and transfer back to their local GP and community support networks. Other parts of the role include overcoming barriers by working with translators or technology, participating in CPD, developing internal and external positive and innovative professional and voluntary sector relationships, reviewing information and providing reports

Main duties of the job

o Provides comprehensive assessment, advice and treatment to people with highly complex needs referred to Community Services in accordance with care pathways for given specialism o Delivers specialist interventions in accordance with care pathways for given specialism

o Actively supports the Team Leader/Advanced Practitioner in the achievement of KPI / CQUIN and related County Council targets, and maintenance of registration with regulating bodieso Takes the lead on specific objectives as directed by the Team Lead and Clinical Team Lead.o Provides supervision and direction to practitioners and support staff within community services, undertaking appraisals and supporting the personal development of colleagueso Contributes to the screening of referrals and allocation to service lineso Contributes to the assessment of complex cases to support the appropriate allocation of referrals o Supports a culture of ongoing learning and development on an individual, team and service basis to ensure high standards are maintained and, where appropriate, contribute to the delivery of training as appropriateo Provide leadership to colleagues acting as a positive role modelThe post holder will have a positive attitude to the care of the service users, be adaptable and resourceful and will be required to work regularly without direct supervision.
