Job summary
North London NHS Foundation Trust's Memory Service provides specialist assessment, diagnosis, and post-diagnostic support for individuals experiencing memory difficulties, including dementia. The service operates across Camden, Islington, Barnet, Enfield and Haringey and is delivered by a multidisciplinary team committed to compassionate, evidence-based care.
We are seeking an experienced memory clinic nurse to join our team. The post holder will work along side other clinicians in the monitoring and management of the post diagnostic patient pathway for the service. Providing clinic review to patients with an existing Dementia diagnosis monitoring medications, supporting diagnostic pathways, and contributing to holistic care planning.
Main duties of the job
Conduct comprehensive assessments of patients in the post diagnostic pathway, including medication and physical health reviews. Liaise with GPs, community teams, carers, and voluntary sector organisations to ensure coordinated care. Participate in multidisciplinary team meetings and contribute to diagnostic formulation and care planning. Deliver psychoeducation and medication reviews to patients and families. Maintain accurate clinical records using RiO and other Trust systems. Support service development initiatives, including audits, training, and pathway redesign.
