Job summary

A fabulous opportunity has arisen for a chance to join team as a band 6 Mental Health Practitioner. The post provides a good work life balance, working 9-5, 4 days a week (30 hours total). We care for Older People based in the coastal communities of Suffolk. Our team is rated Good by the CQC and we are accredited with the Royal College of Psychiatrists under their ACOMHS scheme, and we are proud of the excellent service we provide.

Our friendly and supportive team provides a community-based service to people of any age with a dementia diagnosis, and to Older People with a functional diagnosis and complex needs.

We are looking for someone who is interested in working in Older People's services, has an interest in physical as well as mental health and who has an understanding of assessment, the referral process, case handling and working collaboratively with the wider MDT.

Main duties of the job

As a Community Mental Health Practitioner, will be responsible for:

Holding a caseload of patients with organic and functional presentations under CPA and NCPA Assessing and managing risk. Using Dialog+ where possible and Recovery Care Planning with service users and carers. Managing your own diary and planning your day. Attending MDT meetings and ward review meetings. Duty working 2 half days per week. Communicate effectively through notes on Lorenzo and letters to GPs and other involved parties. Administering depot medication. (for nurses) There is an opportunity to learn to take ECG readings and to learn phlebotomy. (for nurses) Driving is essential for this role. Full induction will be given.

Assessing mental health needs and supporting with physical health. Assessing risk, formulating care plans and safety plans with patients and carers. Taking part in psychological formulation Supporting students on placement. Supervision of junior staff. This post involves driving and seeing patients in their own homes.

Other Essential

Ability and willingness to travel independently to meet the needs and requirements of the service

Qualifications Essential

Registered Mental Health or Learning Disability Nurse, Occupational Therapist OR Social Worker with the relevant professional registration Ongoing professional development working towards degree level ( if professional registration is following a diploma level course)

Desirable

Higher degree or post graduate certificate in area of clinical / therapeutic practice

Experience Essential

Able to demonstrate an appropriate level of knowledge of mental health practice from previous experience Experience in the delivery of clinical interventions

Skills Essential

Competent assessment skills
