Job summary

We are seeking an enthusiastic and motivated qualified healthcare professional (e.g. Nurse, Pharmacist, or Allied Health Professional) with a special interest in diabetes management and insulin initiation to join our friendly and supportive team.

This is a fantastic opportunity to take on a well-established role, following the retirement of a valued and experienced colleague.

You will play a key role in delivering high-quality patient care, with a strong focus on chronic disease management, particularly diabetes, supporting patients through treatment optimisation and insulin initiation, helping us to improve health outcomes for our patient population.

About the Role

Working closely with our Lead Practice Nurse, GP team, and wider multidisciplinary team, you will:

Deliver a wide range of clinical clinics and patient assessments Lead and support chronic disease management, with a focus on diabetes care Support and/or initiate insulin therapy in line with local and national guidelines Contribute to the ongoing development of clinical services and best practice Promote health, wellbeing and preventative care across our patient population Work collaboratively across all three sites

Main duties of the job

Key Responsibilities

Your duties will include (but are not limited to):

Managing and delivering patient care within scheduled clinics Monitoring and supporting patients with long-term conditions (including diabetes, asthma, COPD, CHD, hypertension and CKD) Supporting medication reviews and optimisation (where appropriate to professional background) Providing immunisations (including flu and travel vaccines, where qualified) Performing or supporting procedures such as wound care, ECGs, phlebotomy and health checks (role dependent) Delivering lifestyle advice and structured education, particularly for diabetes Maintaining accurate electronic patient records and contributing to audit Supporting minor surgery and infection control processes You will also have the opportunity to undertake extended clinical duties such as:

Diabetes management, including insulin initiation and titration Supporting newly diagnosed diabetic patients Travel health clinics Child immunisations (where trained) Mental health and learning disability reviews Teaching and mentoring (where qualified)

The Practice and PCN were one of four pilot sites across Devon, offering additional appointment capacity to explore alternative models for managing urgent same day demand.

About You

We are looking for a compassionate and proactive healthcare professional committed to high standards of patient care.

Essential:

Registered healthcare professional (e.g. Nurse with NMC registration, Pharmacist with GPhC registration, or relevant HCPC registration) Minimum 3 years post-registration experience (primary care, community, or hospital) Experience or strong interest in chronic disease management Knowledge of diabetes care and willingness to support insulin initiation Excellent communication, organisational and IT skills Ability to work both independently and as part of a multidisciplinary team Desirable:

Experience in diabetes management and insulin initiation Independent prescribing qualification (or willingness to undertake) Relevant qualifications or training in long-term conditions Training in areas such as immunisations, asthma/COPD, or wound care Mentorship or teaching qualification

Person Specification

Qualifications Essential

Registered healthcare professional (nurse with MNC registration, pharmacist with GPhC or relevant equivalent). Additional training in diabetes management and insulin initiation. Primary Care Experience Minimum 3 years post-registration experience (primary care, community, or hospital). Excellent communication, organisational and IT skills. Ability to work both independently and as part of a multidisciplinary team. Able to manage own workload within departmental priorities and protocols. Able to keep accurate electronic and manual records with appropriate audit trails.

Desirable

Independent prescribing qualification. Knowledge of EMIS. Relevant qualifications or training in long-term conditions. Mentorship or teaching qualification.
