We are looking for a Band 5 Community Physiotherapist to join our UEC team.

The position will be covering the Mid Nottinghamshire area (Mansfield, Ashfield, Newark & Sherwood) . The successful candidate will work within the Mid Notts UEC teams and support the delivery of community rehabilitation services and discharge to assess and admission avoidance.

Main duties of the job

Working within the integrated team, you will be supported to effectively triage, prioritise, assess and provide holistic interventions for clients referred to the service, within their home environment. You will work closely with health and social care colleagues across primary and secondary care to ensure continuity, effective intervention and maximise rehabilitation outcomes.

You will offer bespoke physiotherapy to adults, the frail and elderly including complex fallers, patients with neurological conditions, complex co-morbidities and orthopaedics.

We deliver services between 8am and 10pm over 7 days. We currently operate shift patterns for Band 5 posts between 8 am and 6pm on 5/7 days.

Please note that this post does not meet the pay level required for a Skilled Worker Visa.

Successful applicants with no prior NHS experience would normally be placed at the bottom of the band in line with Agenda for Change. This salary is below the minimum salary required for sponsorship for a Skilled Worker / Health & Care visa. In these circumstances the Trust would not, therefore, be able to sponsor for a Skilled Worker / Health & Care visa.

Job description Job responsibilities

Applicants should hold BSc/Diploma in Physiotherapy, HCPC Registration. You will ideally have post qualification experience and recent experience of working with clients with long term conditions, falls, neurology and health care of the elderly conditions.

A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

Allied Health Professionals within Nottinghamshire Healthcare Foundation Trust are at the forefront of a range of multi-disciplinary, specialist services for people with complex needs, using their expertise around promoting recovery, enablement and positive risk taking, to add life to years, prevent relapse and reduce risks.

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment.

You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies.
