About this role

Job summary

The Intermediate Care Beds Teams provides a time limited period of assessment, treatment and rehabilitation for adults living within Calderdale or registered with a Calderdale GP. Service users may have had an alteration in their physical or psychological well-being, or a temporary change in their social circumstances which makes it difficult for them to be maintained at home. Alternatively service users may have been discharged from an acute hospital setting and require a period of 24 hour supported living, assessment and rehabilitation to enable them to return home.

The post holder be based within one care home but may be asked to work in any care home within Calderdale dependent on the needs of the service.

The post holder will work as part of a multi-disciplinary team comprising Occupational Therapy, Physiotherapy, Assistant Practitioners, Rehabilitation Assistants, Local Authority staff and care home staff.

Person Specification

QUALIFICATIONS / TRAINING Essential

Relevant Degree/Diploma in Occupational Therapy HCPC registered Clinical educator training

KNOWLEDGE, EXPERIENCE & EXPERTISE Essential

Recent experience of assessing, treating and discharging patients with complex needs at a specialist level, and evaluating care. Recent experience of working in the NHS as an Occupational Therapist in a non-acute hospital settings Experience of training, developing and supervising others Experience of contributing to service improvements Knowledge of relevant legislation and local/National initiatives

COMMUNICATION AND RELATIONSHIPS Essential

Teaching/Presentation skills Ability to communicate effectively with people with complex difficulties and in difficult situations Demonstrates positive attitudes and behaviours Self-motivated with ability to work independently, identify own training needs and influence others Ability to work in a stressful environment and concentrate in conditions requiring emotional demands Independent problem solving/decision making skills based on situation and patient needs.
