Job summary
Hampshire and Isle of Wight Healthcare NHS Foundation Trust is seeking a motivated and proactive Smoking Outreach Worker to lead the strategic and operational delivery of smoking cessation initiatives within General Practice settings. This role plays a pivotal part in supporting the Trust's smokefree ambitions and improving population health outcomes across Hampshire.
Working in close partnership with Hampshire County Council Public Health, NHS departments, and wider community stakeholders, you will drive the implementation of tobacco control programmes that strengthen referral pathways, enhance patient engagement, and embed consistent smokefree practices across primary care.
You will take responsibility for developing and maintaining integrated electronic clinical systems, ensuring accurate patient record management, and generating targeted cohort lists to support effective outreach. The role requires a high level of initiative, specialist knowledge, and resourcefulness to coordinate programme delivery, support staff training, and champion new smokefree referral pathways for patients, staff, and visitors.

Main duties of the job
The smokefree outreach worker will promote a clear smoking cessation referral pathway, and be supportive of implanting new systems, collaborative working and sharing learning for practice staff, patients and visitors on-site by:
* Embedding a smoking cessation policy, protocol, and onward referral pathway into GP settings.
* Working closely with partners including clinicians, social prescribers, health coaches, mental health teams, pharmacy colleagues, domiciliary and Hampshire Smokefree advisors to embed a smokefree culture into the GP setting.
* Promoting all available smokefree training.
* Acting as the Smokefree champion, promoting the smokefree message in day-to-day work activities and across the site.
* Providing outreach to recorded smokers in the form of telephone calls, SMS / email communications, NHS App messaging or at events.
* Striving for excellence and aiming for quality of quits, through delivering Smokefree Very Brief Advice interventions, confident conversations
* Where applicable, across primary and secondary care services work with the IT and digital teams to streamline or integrate a smoking cessation referral template into the clinical system (Emis, SystmOne, Accurx, iPlato, Pharmoutcomes etc).
* Supporting quarterly and annual evaluation(s) for the above-mentioned activity and service outcomes.

Person Specification
Qualifications Essential
Educated to GCSE (or equivalent) grade C / level 4 standard pass or above in English and Maths. Commitment to obtain underpinning knowledge through work based learning and mandatory training.
Desirable
Specific training in health promotion interventions National Centre for Smoking Cessation and Training -- Practitioner Training / Very Brief Advice+ / Swap-to-Stop specialist modules, Motivational interviewing Knowledge of NRT used in treatment
Experience Essential
Experience of working in Health and Social Care setting or community Experience of providing healthcare, healthcare advice or health improvement/promotion advice to patients or members of the public Experience of using Microsoft Office software
Desirable
Experience of delivering stop smoking brief interventions Experience of providing help, behavioural support and/or counselling on a one to one or group basis Experience of working in a health improvement related field Experience using EMIS Previous smoking cessation lived experience
Additional Criteria Essential
