About this role

Job summary

The post holder will provide high-quality, patient-centred acute frailty care within the Humber Virtual Frailty Ward, working closely with the Community Frailty Hub to avoid unnecessary hospital admissions and support safe care in patients usual place of residence. The role involves autonomous and multidisciplinary working to assess, treat, and manage patients with acute frailty-related deterioration, including falls, delirium, mobility decline, deconditioning, medication concerns, and exacerbations of long-term conditions.

Responsibilities include completing frailty assessments and Comprehensive Geriatric Assessments (CGAs), developing timely management plans, making safe clinical decisions in line with NICE and GMC guidance, and coordinating rapid community interventions. The post holder will provide senior clinical input to virtual ward MDT discussions, support admission avoidance and early supported discharge pathways, and assist with proactive escalation and advance care planning, including ReSPECT and Treatment Escalation Plans.

Working across primary care, community services, secondary care, ambulance services, mental health, social care, and voluntary organisations, the role supports a no wrong door approach to integrated frailty care focused on recovery, independence, and preventing unnecessary hospital attendance. The post holder may also contribute to clinical governance, audit, service evaluation, and quality improvement initiatives.

Main duties of the job

We are seeking a motivated and experienced clinician to join an excellent multidisciplinary team, providing high-quality, patient-centred care to vulnerable and frail patients in the community. The role focuses on clinical assessment, care coordination, and supporting patients to remain safely at home wherever possible.

Person Specification

Qualifications Essential

Full and current unrestricted GMC (UK) registration on the GP Register A medical degree qualification Up to date knowledge of all the latest guidelines and evidence in older peoples medicine Understand the importance of evidence-based practice and clinical effectiveness Understanding of clinical governance Sound clinical knowledge Ability to use the evidence base and clinical audit to support decision-making

Desirable

A post graduate qualification in older peoples care, e.g. Diploma Geriatric Medicine (or willing to work towards, we will support you to attain this and fund this) Experience of working a variety of clinical software systems

Skills and Competencies Essential

High level of communication skills Good organisational skills IT competency Ability to make good clear concise medical notes, both computerised and manual Able to work under pressure and within an ever-changing environment Ability to motivate and develop the multi-disciplinary team Organised and efficient in record keeping and completion of paperwork Time management skills -being able to prioritise work and work under pressure Computer literacy

Experience Essential

High level of clinical skills. Successfully Completed General Practice Specialist Training Appropriate range of previous experience in hospital posts Ability to teach clinical skills to medical and nursing staff and other disciplines.

Desirable

Desirable to have completed at least 2 years of GP specialist training Experience of System One Experience of working within a Virtual Ward service or within urgent community response care
