Job summary
Band 3 Community Support Worker We have an exciting opportunity to join our well-established Herefordshire Memory Service. The Support Worker will coordinate and support a range of post-diagnostic support interventions/ groups, which include Cognitive Stimulation Therapy. While supporting nurses with medication management. The Support Worker will be working within the multidisciplinary team.
The service operation between the hours of Monday to Friday, 9 to 5, excluding bank holidays. Flexible working patterns will be considered. The team is based in the Benet building at Belmont, in the grounds of the Belmont Abbey, which is a peaceful rural location, while being 2 miles from the main city of Hereford. There is plenty of space provided for free parking. The team works between a split of office-based work and working from home in line with the service requirements and personal preferences.
Please note the Trust will not routinely provide sponsorship to work for this role. Employment on a time limited visa doesn't guarantee sponsorship at point of expiry.
Main duties of the job
Provide care and support as directed by the Community Service in response to individual patient need within the designated caseload and with a particular focus upon therapeutic and group work.
Participate when necessary in initial assessments with members of the Locality Team.
Promote maximum patient independence at all times.
Participate in health promotion initiatives.
Monitor the patients level of functioning with activities of daily living, offering assistance and support as necessary.
Where applicable, monitor and report the service users level of functioning within groups and sessions within the therapeutic programme offered.
Provide active support to the carers of patients on case load.

Support professionally qualified staff, often without direct supervision, to ensure that patient and carers within the designated caseload receive, through liaison with the appropriate agencies, a range of services and resources appropriate to their needs.
Ensure Recovery focussed patient care interventions and care planning.
Participate and support the Community Services other professional duties.
Ensure written records are kept up to date to facilitate evaluation and monitoring of all patients.

Skills and Abilities Essential
Ability to challenge poor behaviour. Role modelling good behaviour. A positive attitude and able to act on feedback. Ability to praise and be supportive to others. Ability to communicate clearly and effectively in English in both verbal and written formats.
Qualifications Essential
Level 3 qualification (e.g., A-level, NVQ3) in Care or equivalent qualification or experience. Level 2 qualification (e.g., GCSE, NVQ2) in Care or equivalent qualification or experience and will be expected to undertake the relevant level 3 qualification on appointment to post. Will be expected to meet the Care Certificate standards within 12 weeks of commencing employment with the Trust. I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.

Dementia/mental health training.
Experience Essential
Previous relevant experience working with the client group. One to one working. Personal care tasks.

Knowledge Essential
The post holder will demonstrate a sound knowledge and understanding of Communication skills, Literacy skills, Numeracy skills , Advocacy skills, Observing skills, Recording skills & Liaison skills.
Additional Criteria Essential
Applicants should be able to work flexibly and with minimal supervision. The ability to cope with changing situations to routine is also important. Able to meet the travel requirements of the post.
