Working alongside adult social care, you will be responsible for the delivery of integrated, accessible and strengths-based interventions for adults with complex needs and substance use concerns. You will need to focus on safeguarding, risk management and positive outcomes with the aim of reducing the need for costed adult social care packages.

Main duties of the job

Hold a caseload of clients with multiple needs. You will attend and contribute to multi agency meetings. You will also be required to update client records in a timely manner and complete formal reports. You will provide therapeutic and outreach support, including home visits and community-based work, and supporting access to work-related activities. You will work closely with social workers and multi-agency partners to carry out assessments, risk assessments, reviews and complete support plans, ensuring a person-centred approach.

To deliver assertive interventions that will be fully integrated, accessible and targeted to adults with complex needs that result in positive outcomes To have good knowledge of complex needs, specifically trauma and its impact, substance misuse models, mental health presentations, risk behaviours, safeguarding legislation and behaviour change theory. To deliver an intensive, strength-based model to adults with complex needs, in their home. This may include safety planning, strengths, values, goals and tailored sessions for the individuals specific needs To manage a complex caseload with multiple, interacting needs and make safeguarding decisions To work holistically, focusing on the key concerns for the adult To handle complex, sensitive information in line with policies and procedures, and confidentiality To attend and contribute to strategy meetings, planning meetings, review meetings and multi-agency meetings for the adults being supported To work closely with the allocated social worker, providing regular telephone/email updates, formal written reports and working towards agreed goals and outcomes. To liaise with all agencies working with the individual

Person Specification

Qualifications Essential

A degree in Social Work, Psychology, Criminology, Health and Social Care or a related field is required due to the complexity of the role Level 2 safeguarding trained, as a minimum Good general level of education and evidence of continuing professional development

Skills Essential

UK driving licence

Experience Essential

Previous experience of working with adults with complex needs Experience and knowledge of supporting vulnerable people and utilisation of structured needs / risk assessments
