As a care coordinator, you will be the first point of contact for our practice, welcoming patients, managing appointments and supporting the smooth day-to-day running of our busy surgery.
Main duties of the job
We are seeking a friendly, organised and professional GP care navigator to join our team. As the first point of contact for patients, you will deliver a welcoming and efficient service both in person and over the phone. Your responsibilities will include booking and managing appointments, responding to patient enquiries, updating records and providing general administrative support to the clinical team.
The successful candidate will demonstrate excellent communication skills, a calm and compassionate approach, strong attention to detail and the ability to work effectively under pressure in a busy practice environment.
Previous experience in a GP setting is essential.

The post holder will be the first point of contact for patients, both in person and by telephone, providing a welcoming, professional and efficient reception service. The role involves supporting patients to access appropriate clinical care while maintaining a friendly and compassionate approach.
Key duties:
Greet patients and handle enquiries courteously in person and by telephoneBook, cancel and reschedule appointments including requests for home visitsTriage patient enquiries and pass messages accurately in line with practice proceduresKeep patients informed of delayed and address concerns sensitivelySupport GPs, Nurse and the wider clinical team with administrative requestsOrder ambulances and interpreters as requiredMaintain familiarity with appointment and telephone systemsWork effectively as part of the reception teamHandle complaints in accordance with practice proceduresEnsure reception and waiting areas are clean and tidy
