Job summary
The Transfer of Care Hub (ToCH) is a Service which has been developed to implement an expert complex multi-disciplinary and inter-disciplinary discharge team, working in a seamless and integrated way across partner organisations, both health and social care. The ToCH will proactively 'pull', and case manage the needs of a range of people with complex discharge needs on discharge pathways 1-3 and progress these people safely to transfer/discharge via an appropriate pathway
The Transfer of Care Hub (ToCH) Lead Therapist (Band 7) is a senior clinical leadership role responsible for delivering high-quality, timely and patient-centred discharge planning within a fast-paced acute setting. Acting as the therapy lead within the multidisciplinary Transfer of Care Hub, the post holder provides specialist clinical expertise, leads the therapy contribution to Discharge to Assess (D2A) pathways, and supports safe, effective patient flow across health and social care systems.
The role involves close collaboration with internal teams and external partners, providing leadership, supervision and professional guidance to the MDT, while contributing to service development, governance, and performance improvement. The post holder will play a key role in reducing delays, improving patient outcomes, and ensuring discharges are coordinated efficiently and in line with national and local priorities.
Main duties of the job
Lead the therapy contribution within the Transfer of Care Hub, supporting safe, timely and effective patient discharge. Provide highly specialist clinical assessment, decision-making and advice for patients with complex needs. Act as the first point of escalation for therapy-related discharge issues, resolving barriers and supporting patient flow. Lead and coordinate the Discharge to Assess (D2A) pathway in line with national guidance. Work collaboratively with multidisciplinary teams and system partners across health and social care. Provide clinical leadership, supervision, training and support to therapy staff and the wider team. Contribute to service development, clinical governance, audit and quality improvement activities. Support performance management by monitoring discharge metrics and addressing delays in care pathways. Maintain a clinical caseload and work autonomously within professional scope.
