What to expect: Direct and provide social services to meet the facility residents' psychosocial needs.

Successful candidates: Bachelor’s Degree in Social Work or in Human Services - MSW preferred 2 years of supervised social work experience in a health care setting working directly with individuals. Experience in a post-acute or skilled nursing facility a plus!
Rate: $31.25-$36.06/hour
